Proven, Sustainable Approach to Expanding DPP to Counties Hardest Hit By Type-2 Diabetes Across Missouri, Ohio, Illinois, and Virginia
HabitNu, a national provider of the National Diabetes Prevention Program (NDPP) with full CDC recognition, a proven CDC-recognized DPP Umbrella Hub management strategy, in-house technology for participant engagement, data management, billing, and claims submission expertise seeks to deploy our Umbrella strategy to scale and sustain the DPP. HabitNu will lead a multi-state, multi-sectoral partnership of stakeholders, and serve as a conduit for bi-directional collaboration and improvement initiatives between state departments of health, referral organizations, local DPP communities, lifestyle coaches, payers, and employer groups. HabitNu will offer our Umbrella Hub administration expertise, DPP platform, and network access to DPP providers (“subsidiaries'') in the HabitNu Umbrella. We will focus our efforts in four key states in which we are already active: Missouri, Ohio, Illinois, and Virginia, and will target minority, African American, and rural communities that face health inequity and food insecurity.
To address the social determinants of health (SDOH) needs of priority populations and expand the participant recruitment reach of the Umbrella, HabitNu will collaborate with Unite Us, a national SDOH platform with an established network of health and social service providers and care navigators. HabitNu’s strategic vision is to break down silos and bring long-term financial sustainability to DPP delivery by deploying integrated Umbrella networks across each state. The goal of these networks will be to promote broader payer coverage and remove obstacles to effective DPP delivery by connecting Component A awardees (state government), community-based Component B awardees, and new and existing DPP providers.
