New Mexico Department of Health Diabetes Prevention and Control Program Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes - Project Abstract Summary The New Mexico Department of Health (NMDOH), Population and Community Health Bureau, Diabetes Prevention and Control Program (DPCP) submits this proposal under Component A of CDC-RFA-DP-23-0020: A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes (RFA 23-0020). NMDOH is a centralized state health department serving New Mexico’s 33 counties and 22 sovereign tribal nations. DPCP proposes to serve the entire state with this proposal. Under this funding stream, DPCP aims to address health disparities in diabetes by reducing social determinants of health (SDOH) related barriers impacting the priority populations’ successful access to and participation in evidence-based diabetes management and type 2 diabetes programs and services. New Mexico has prioritized reach to these priority populations: Hispanic/Latino, American Indian/Alaska Native, and African American/Black-especially if they earn at or below the federal poverty line (FPL) and/or live in rural communities with limited resources. In a majority-minority state like New Mexico, when we address groups who have systematically experienced greater obstacles to health based on their race or ethnic group, we concurrently achieve statewide reach. To address health disparities in diabetes in New Mexico and achieve statewide reach, DPCP will implement the following six strategies: 1, 5, 8, 9, 12, and 13. Collectively, we anticipate each strategy to build on prior successes under 1815, reach our priority populations, and achieve our 5-year outcomes (as well as the short and intermediate outcomes identified in the CDC logic on pages 5-7 of RFA 23-0020). DPCP has the capacity—through staff and partnerships—to serve all populations and communities within the state and, particularly, the identified priority populations. To oversee and manage this project, with its numerous partnerships and collaborations, DPCP will engage a leadership team comprised of those most central to envisioning, implementing, and sustaining this project. We expect this team to include DPCP, Heart Disease and Stroke Prevention Program (HDSPP), the project evaluator, and contractors outside of the NMDOH including, Association of Diabetes Care and Education Specialists (ADCES), New Mexico State University (NMSU) Extension Office, Organizational Rebel, Comagine Health, Constellation Consulting, and Project ECHO. To reach priority populations and enable them to successfully access and participate in diabetes-related programs/services in New Mexico, DPCP is, first, focusing work in the geographic areas and communities with the highest rates of diabetes to ensure we offer services where people live. Second, we will continue to collaborate with accredited National DPP and DSMES sites (e.g., NMSU Extension Services) who have the infrastructure and experience to reach New Mexico’s priority populations. As the project evolves, DPCP plans on having partners who have successfully implemented their work guide and support new programs as they seek to establish themselves and reach priority populations. Third, DPCP intends to continue working with the NMDOH Office of Community Health Workers (OCHW) to train and provide continuing education opportunities to community health workers (CHWs), community health representatives (CHRs), and promotoras on diabetes care, management, and prevention. Fourth, to address the implementation of evidence-based, family-centered childhood obesity interventions as a type 2 diabetes risk reduction strategy, we will work with Organizational Rebel to identify and convene clinical and community stakeholder representatives of the priority populations to select one of the CDC-identified evidence-based, family-centered childhood obesity interventions.
