Connecticut: School-Based Interventions to Promote Equity and Improve Health, Academic Achievement, and Well-Being of Students - The purpose of this 5-year cooperative agreement is to support schools and districts in using the Whole School, Whole Community, Whole Child (WSCC) model to protect and improve the health and well-being of school-age children and adolescents. This cooperative agreement focuses on improving health outcomes for children and adolescents in medically underserved communities and communities that are disproportionately affected by chronic diseases and attributable risk factors. The UConn team will work with the State of Connecticut’s Department of Education, Department of Health, and a priority LEA: East Hartford Public Schools, to increase implementation of evidence-based school health policies, practices, programs and services. The UConn team will establish key partnerships to support school health programs; provide professional development (PD) and technical assistance (TA) to promote use of the CDC’s school health guidelines, assessment tools, and resources; and guide schools and districts in action planning to strengthen school health policies, practices, programs, and services.
We will achieve the following outcomes: (1) an increased number of schools with an action plan based on the completion of CDC’s School Health Index (SHI); (2) increased implementation of Comprehensive School Physical Activity Programs (CSPAP); (3) increased school nutrition and food security policies and practices; (4) increased school-based health services that support students with chronic health conditions; and (5) increased physical activity, and healthy dietary behaviors. These outcomes will be measured or captured using CDC’s Schools Health Profile, School Health Index, and Youth Risk Behavior Survey.
