As a newly invited HP2030 Champion Health Department, Tarrant County Public Health will collaborate with John Peter Smith Hospital System (our county hospital district), North Texas Area Community Health Clinics (the local Federally Qualified Health Clinic), Southside Church of Christ (Faith-based), LGBTQ SAVES (Mental and Social Services hub for LGBTQ+ youth), the Dallas Fort Worth Community Health Workers (Community-based Organization), and at least five other multisectoral agencies to develop an implementation ready Social Determinants of Health Accelerator Plan (SDOH). This Accelerator Plan will use HP2030 and CDC guidance to focus on reducing disparities in Social Determinants of Health Priority Areas Community-Clinical Linkages, and Social Connectedness.
The SDOH Accelerator Plan will be designed to improve chronic disease outcomes by increasing access to, and utilization of preventive services located within the identified zip codes. The Plan will also include strategic methods to protect, improve and maintain individual and community well-being by reducing the negative health effects of psychological adversities such as stress, adversity, and trauma by increasing the likelihood of individuals engaging in positive health behaviors within a minimum of 4 of the 16 high health equity zip codes within Tarrant County.
It is our intent that this NOFO will allow us to increase collaboration and engagement across multisectoral partners. We will convene and formalize a newly established Leadership Team that will serve to develop and implement the SDOH Accelerator Plan. As evidenced by our letters of support, our partners are from an array of sectors, and have the assets, experience, and desire to make a favorable impact within our community. We continue to will expand our partnership through the identification of sectors that can build our capacity to serve the community through the SDOH Accelerator Plan.
Upon the successful coordination of the multisectoral Leadership Team, the local JPS CHNA, Health Equity Index, American Community Survey, and other data sources will be used to develop a comprehensive SDOH Accelerator Plan that will focus on the SDOH Priority areas of Community-Clinical Linkages, and Social Connectedness within a minimum of 4 of the 16 high health equity index zip codes identified within Tarrant County.
