In response to the CDC announcement of funding to develop multisector action plans to address social determinants of health SLMC is drawing on historically strong community collaboration to close inequities identified in our 2021 Community Health Assessment. Of the CDC’s five determinants that impact chronic disease risk factors and health outcomes SLMC will focus on Built Environment, Community-Clinical Linkages, and Food and Nutrition Security. In a landscape populated by some of Oregon’s most vulnerable citizens, a single agency in Klamath County would be unable to influence population health issues and trends. There is a strong history of collaboration in the best interest of the community’s health equity, and that focus is increasingly broadening to influence the overall environment in Klamath County. SLMC has leveraged the alignment of resources and missions throughout the community to improve population health featuring four lead agencies — Cascade Health Alliance, Klamath County Public Health, Klamath Health Partnership, and SLMC. These successful partnerships with be strengthened by incorporating both community members most impacted by health disparities and expanding representation from other sectors, including high education and food markets into the development of the SDOH Accelerator Plan.
