The Lackawanna County Department of Health and Human Services (LCDHHS) and its associated agencies, in partnership with the Center for Health & Human Services Research & Action (CHHSRA), plans to address economic stability, health care access and quality, and social and community context to help advance the CDC’s public health mission to help keep Lackawanna County safe and healthy where they live, work and play. The applicants propose to maximize the area’s public health impact, policy, systems, and programmatic strategies that address social determinants of health by narrowing disparities in chronic diseases, increase health equity, and encouraging the practice of healthy behaviors to the area’s most vulnerable populations. This proposal entails providing mobile physical and behavioral health screening and testing to vulnerable populations, as well as a social and well-being screening, and facilitating immediate treatment and services when indicated. The pilot project will work with families who are involved with the county child welfare system and who are homeless and living in temporary housing in hotels provided by the county.
