The Philadelphia SDOH Accelerator Plan - Philadelphia is a large, diverse city facing heavy burdens of poverty, chronic disease, and inequality. The Philadelphia Department of Public Health proposes to convene and coordinate a multisectoral team of stakeholders in the Philadelphia community to address social determinants of health within the built environment and community-clinical linkages. The PHL SDOH Leadership Team includes the Health Department, other City agencies, established task force and advisory councils, healthcare systems, community-based organizations, community legal services, the Philadelphia Health Information Exchange, and non-profit experts in public health program delivery, quality improvement, and service coordination. Plans will be developed in partnership with communities that face disparities in chronic disease and related risk factors—Black and Hispanic Philadelphians and six geographic regions of the city: North Philadelphia East, North Philadelphia West, Far North Philadelphia, the River Wards, West Philadelphia, and Southwest Philadelphia whose needs were highlighted in the 2022 Philadelphia Community Health Needs Assessment.
With the aim of developing a sustainable and implementation ready SDOH Accelerator Plan the PHL SDOH Leadership Team will 1) define the scope of disparities in chronic disease outcomes and the social determinants of health in Philadelphia, 2) identify and map existing community assets and ongoing programs operated by the SDOH Leadership Team partners, 3) include and respond to the input and collaboration of community partners, 4) identify gaps in assets and services, 5) coordinate efforts, 6) develop new strategies to address these inequalities, 7) align partner resources and capacities, and 8) plan to evaluate the impact of this work.
