The Healthy Communities Coalition’s proposed project aims to prevent and reduce the impacts of chronic disease by creating an accelerator plan to combat social determinant of health (SDOH) issues in Lewis and Clark County, Montana.
The Healthy Communities Coalition (Coalition) coordinated by Lewis and Clark Public Health has a rich history of collaboration and action. The Coalition members examine programs and conditions already existing in the community and seek to identify opportunities for improved policy, system, and environmental change. The diverse membership on the Coalition includes city, county, and state government; non-profits; local businesses; health organizations; and individual residents in target populations.
The Coalition has accomplished numerous system-wide changes, including but not limited to: Tobacco Free Parks, Tobacco Point of Sale ordinances, Complete Streets Policy, sun safety campaigns, sun hats for newborn babies strategy, and an Active Living Wayfinding Plan.
Using the funding opportunity from the Centers for Disease Control and Prevention’s Closing the Gap with Social Determinants of Health Accelerator Plans program, Lewis and Clark Public Health will create a strong, inclusive, and interdisciplinary leadership team for the Coalition. This will lead to a more impactful, active, and sustainable coalition.
Once established, the Coalition leadership team will hire a consultant to help develop a SDOH Accelerator Plan. This plan will lead the Coalition to a meaningful action plan based on the data and needs. The leadership team will also organize a training for the entire coalition to better understand SDOH and health equity. The target population for the SDOH Accelerator Plan will the County’s most vulnerable populations that are most likely to experience health disparities, including rural residents, American Indians, seniors, LGBT, and people living with a disability.
Purpose
A multisectoral Leadership Team made up of 10 leaders from an interdisciplinary and inclusive backgrounds will work together to develop a SDOH Accelerator Plan to address the priority areas of Community-Clinical Linkages, Built Environment, and Food and Nutrition Security. The target populations for the SDOH Accelerator Plan are rural residents, American Indians, seniors, LGBT, and people with disabilities.
Outcomes
1. By September 30, 2023, the Healthy Communities Coalition will have increased collaboration and engagement across a ten-person, multi-sector Leadership Planning Team.
2. By September 30, 2023, the Healthy Communities Coalition will have a completed, implementation-ready SDOH Accelerator Plan that includes all required components.
