Project Summary: Minneapolis Health Department The Minneapolis Health Department and its partners will reduce chronic disease risk factors in the African American/East African and American Indian populations by implementing nutrition, physical activity and community-clinic linkage strategies that take into account their respective housing patterns and integrate their cultural preferences. In each community, they will combine a people-centered approach that supports individuals through education and engagement with a place-based approach that creates environments that foster good health. Venues and Partnerships The African American/East African community will be engaged through 55 large, multi-unit public/subsidized buildings owned and operated by CommonBond, Minneapolis Public Housing and Volunteers of America. For the American Indian community, REACH strategies will be embedded in community-based, cultural organizations and the neighborhood in which they are located. Main partners include the Native American Community Development Institute, Division of Indian Works, Dream of Wild Health and the MN American Indian Center. Strategies and Activities Nutrition: The Health Department and its partners will increase and improve healthier food options for residents at housing properties and in the American Indian community through healthier vending, congregate dining, mobile food markets, community gardening and an indigenous food system that will provide school meals. American Indian agencies will expand breast feeding support by establishing a monthly Baby Café and by creating a system for referring new mothers to native women trained to provide breastfeeding support. Physical Activity: Minneapolis Public Works, Native American Community Development Institute and housing providers will lead residents of each community in walking audits to identify pedestrian safety issues and advocate for improvements. Public Works will also engage residents in planning for upcoming street projects to ensure they meet residents’ mobility needs. Community Clinic Linkages: Housing providers will increase the delivery of chronic disease prevention programs in the REACH housing buildings and integrate community health workers and navigators to link residents to clinics and other services. Outcomes Through these venues and strategies, the Health Department and its partners will: • Reach 27,362 housing residents (79% of whom are African American/East African and 4000 American Indians for a total of 31,362 Minneapolis residents. • Increase the number of multi-unit housing properties and community agencies with new or expanded opportunities for breastfeeding support and healthy food access. • Increase the number of infrastructure improvements made and scheduled that integrate residents’ needs and result in safer, accessible places for physical activity. • Increase residents’ participation in on-site or community-based chronic disease prevention/management programs.
