The Connecticut Department of Public Health (DPH) is requesting $900,000 from the federal Centers for Disease Control and Prevention (CDC) to enhance the Connecticut Colorectal Cancer Control Program (CRCCP), which will provide colorectal cancer (CRC) screening, diagnostic, and treatment referral services. Program activities funded under this program include increasing CRC screening services to target communities with low CRC rates; providing quality, appropriate and timely CRC screening, diagnostic, case management, patient navigation, and treatment referral services; expanding partnerships with two health systems [Hartford Health Care (HHC) and Yale New Haven Health (YNHH)], consisting of ten hospitals and Federally Qualified Health Centers (FQHCs) and clinic partners; expanding an existing partnership with an organization that provides expertise to support to the implementation of evidence-based interventions (EBIs) in primary care clinics (i.e., SMC Partners); conducting formal assessment of each clinic’s capacity/readiness to collect high-quality clinic data and implement EBIs; implementing at least two EBIs in primary care clinics in populations with low CRC screening to increase clinic-level screening rates; providing a resource to partner clinics and supporting strategic policy, systems, and environmental changes that decrease CRC burden; collecting and submitting high-quality clinic-level data (baseline and annual) on CRC screening; and, enhancing dissemination and use of cancer data for state planning. The CRCCP will be integrated with the Connecticut Early Detection and Prevention Program (CEDPP) which consists of the Connecticut Breast and Cervical Cancer Early Detection Program (CBCCEDP) and the Well-Integrated Screening and Evaluation for Women Across the Nation Program (WISEWOMAN). Using the established infrastructure for the CEDPP, two large contracted health systems and their network partners will deliver CRCCP services. Activities will be directed in seven Connecticut communities whose CRC age-adjusted incidence rates for 2010-2014 were significantly higher than the state rate based on an analysis conducted by the Connecticut Tumor Registry. These are as follows: Bridgeport (males); Cromwell (males); Hartford and East Hartford (females and both sexes combined); Killingly (females and both sexes combined); New Britain (both sexes combined); Waterbury (males and both sexes combined); and, Windsor (females). Based on this analysis, DPH has chosen to target EBIs in these communities which are scattered across the state. Priority populations in Connecticut will be those that experience health disparities in health outcome and inequity in health care access. This has necessitated Connecticut to target approaches of intervention to those who live with lower income, i.e., Connecticut residents with low educational attainment and earning potential; those with no health insurance coverage; men and women of color; LGBTQ community; and, residents who do not receive routine, preventive health screenings. Through partnerships and collaborations, the CRCCP will intensify CRC activities via implementation of EBIs within two large health systems with multiple clinics and other settings. It is anticipated that all Connecticut residents with or without health care insurance benefit from these actions, but especially those individuals are not well connected to health care services. With screening and early intervention, CRC can be diagnosed and treated successfully.
