Colorectal cancer (CRC) is the third most common diagnosed cancer and the second leading cause of cancer deaths in Oregon, taking about 300 lives per year . Since 2009, the Health Promotion and Chronic Disease Prevention (HPCDP) section of the Oregon Health Authority’s Public Health Division (OHA-PHD) has been funded by the US Centers for Disease Control and Prevention (CDC) to work with clinics, health systems, and Medicaid delivery organizations, along with multiple technical assistance partners with substantial CRC screening and clinic quality improvement expertise to reduce the burden of CRC in Oregon by increasing CRC screening rates for all communities in Oregon, particularly those communities with relatively low screening rates. In Oregon, these priority populations include American Indians/Alaska Natives, Latinos, people on Medicaid, and people living in rural communities. Over the past ten years, the Oregon Colorectal Cancer Control Program (OCRCCP) has placed increasing emphasis on not only increasing overall CRC screening rates in Oregon, but also on reducing disparities between different communities in Oregon. The communities that OCRCCP has prioritized—and will continue to prioritize in the next project period—include Latinos, people on Medicaid, people in rural communities, and American Indians/Alaska Natives. To increase screening rates among these groups and among all people in Oregon, OCRCCP has developed a targeted approach to working with clinics who serve high proportions of these priority populations that relies on working with key partners to identify and engage key clinics and then help them implement clinic-level evidence-based interventions (EBI) for increasing screening rates. With this funding, OCRCCP will continue to partner with health systems and individual primary care clinics with low CRC screening rates to implement evidence-based interventions (EBIs) serving high proportions of priority populations to increase CRC screening uptake and rates of follow-up colonoscopies. Priority populations include American Indians/Alaska Natives, Latinos, Oregon Health Plan (Medicaid) members, and rural Oregonians. Through these partnerships, OCRCCP will continue to not only increase CRC screening rates for the general population but will also reduce disparities in priority population CRC screening rates. Specifically, OCRCCP and its partners will: --Establish partnerships with health system/primary care clinics --Establish partnerships to support implementation of EBIs --Conduct implementation readiness assessments of partner primary care clinics --Implement EBIs (evidence-based interventions) --Facilitate patients’ linkages to follow-up colonoscopy --Gather, report, and analyze high-quality data for program monitoring and evaluation By the end of the project period, OCRCCP will increase clinic level CRC screening rates for all people in Oregon and will also reduce disparities in screening rates for Latinos, American Indian/Alaska Natives, Oregon Health Plan members, and rural Oregonians. OCRCCP will also increase rates of follow-up colonoscopies for patients with positive or abnormal fecal or other non-colonoscopy CRC screening tests. The population served by this project is the 1.2 million people in Oregon age 50-75.
