The Florida Department of Health (FDOH) proposes to successfully implement public health and health systems partnerships to increase colorectal cancer screening in clinical settings among disparate populations across the state within health systems primary care clinics (HSPCCs). FDOH will leverage the strength of existing partnerships among public and private health systems in geographically diverse regions, prioritizing at-risk populations as identified by HSPCC patient data. The FDOH Colorectal Cancer Control Program (CRCCP) is a proven framework health system change, grounded in proven methodology and evidence-based interventions (EBIs) from the Community Guide. Since 2015, FDOH has refined its model of working with health systems to plan and implement EBIs to increase CRC compliance. FDOH will enhance existing and establish new health system partnerships with safety net hospitals, Rural Health Networks and university health systems. The CRCCP model consists of completing an organizational assessment, determining baseline screening rates, selecting evidence-based interventions, developing an implementation plan, and engaging in ongoing monitoring, reporting and evaluation. FL CRCCP HSPCC partners will receive training in quality improvement and colorectal cancer, ongoing technical assistance and participate in regularly scheduled peer to peer shared learning sessions. By the end of the project period the following outcomes will be achieved: • Increased partnerships with health systems and primary care clinics. • Increased and/or improved adoption of multiple EBIs within health care system primary care clinics, insurers, and others. • Increased rates of high-quality screening among defined patient population.
