The South Dakota Department of Health (SD DOH) is applying for CDC-RFA-DP20-2002, Public Health and Health Systems Partnerships to Increase Colorectal Cancer Screening in Clinical Settings. In 2016, colorectal cancer (CRC) represents the fourth leading cause of cancer death and fourth most reported cancer among men and women in South Dakota. The up-to-date screening rate in SD is similar to the national rate, with only 69.1% of the recommended population up-to-date with CRC screening in SD vs. 68.8% in the U.S. (SD BRFSS 2018). This represents approximately 79,700 adults that are not up-to-date with CRC screening in SD. The project purpose is to partner with identified health system partners representing ten primary care clinic sites and three additional collaborating partners to implement evidence-based interventions (EBIs) recommended by The Community Guide to increase clinic-level CRC screening rates. EBIs that will be implemented in partnership with the identified primary care clinics include provider reminders, provider assessment and feedback, client reminders, and reducing structural barriers. The SD DOH has defined the state of South Dakota, with statewide implementation, as the geographic area for program implementation. The target population for this project includes average risk men and women ages 50-75 in SD who are eligible for CRC screening as recommended by the U.S. Preventative Services Task Force. To reduce disparities and achieve maximal impact, the SD DOH will prioritize the following sub-populations: rural and frontier populations, people who are uninsured, people with lower annual household income, people with lower educational attainment, American Indians, individuals age 50-64 years, and men. By partnering with SD health systems and participating primary care clinics to implement the recommended EBIs, the SD DOH will achieve increased clinic-level CRC screening rates, which will contribute to attainment of the intermediate and long-term outcomes identified in the funding opportunity.
