In Kansas, colorectal cancer is the second leading cause of cancer death among men and the third leading cause of cancer death among women. For men and women ages 50-75 years of age, colorectal cancer screening is a preventive measure to reduce colorectal cancer incidence and mortality. However, there are disparities in the screening rates for some groups of Kansans - significantly fewer residents living in rural counties, or those who are uninsured or Hispanic met the United States Preventive Services Task Force (USPSTF) guidelines for colorectal cancer screening (Kansas Behavioral Risk Factor Surveillance System, 2018). In addition, the 2013-2017 colorectal cancer mortality rate is higher among Black as compared to White Kansans (SEER Stat Fact Sheets, accessed 2/4/2020). The USPSTF recommends four interventions within clinic settings to increase colorectal cancer screening rates: assessment and feedback, physician reminders, patient reminders, reducing structural barriers. The Kansas Colorectal Cancer Screening Program will partner with two healthcare collaboratives to support implementation of these interventions within 15 clinics in Year 1. Clinics are located in rural counties, or serve uninsured, Hispanic or Black populations in rural or urban areas. Community health workers and patient navigators will navigate patients to help reduce barriers to cancer screening, completion of follow-up colonoscopies and needed resources for treatment. Some funds will be available for follow-up colonoscopies as payer of last resort.
