The Tribal CRC screening Project will address CRC health disparities among Michigan Native Americans with lower than average CRC screening rates and increased risk factors residing in rural areas of the state. The project will utilize a Community of Practice among four tribal communities in the target population and provide expertise to support the implementation of evidence based interventions, quality improvement processes, patient navigation and quality health data specif to Tribal Health Clinics and I.H.S .systems. Outcomes include increased capacity, increased CRC screening rates and reduced mortality due to CRC, along with sustainable health systems changes necessary to affect long term health improvements for Michigan Tribal Communities. The proposed project builds upon successes and lessons learned from the recent Tribal Colon Cancer Collaborative, which resulted in increased CRC screening rates, increased incidence and earlier stage of diagnosis.
