To increase colorectal cancer (CRC) screening rates in Washington, the Department of Health’s (DOH’s) Breast, Cervical and Colon Health Program (BCCHP) is requesting $900,000 to implement CDC’s Public Health and Health Systems Partnerships to Increase Colorectal Cancer Screening in Clinical Settings funding opportunity. We anticipate serving over 63,000 age eligible clients. We will work with Federally Qualified Health Centers (FQHCs) to implement evidence-based interventions (EBIs) and provide access to follow-up colonoscopy after a positive FIT to uninsured and underinsured eligible clients of health systems implementing EBIs over the five year grant period. In Washington State, Colorectal cancer (CRC) is the second leading cause of cancer deaths with a death rate of 12.4 per 100,000. CRC is the fifth most common cancer occurring in Washington State accounting for 7.1% of all cancers. In 2017, 2,964 residents of the State were diagnosed with CRC and 1,045 residents died of this disease. The national five-year localized survival rate for CRC is 90 percent; however, only 34.3% of cases in Washington were diagnosed at this stage. Adoption of widespread use of effective screening has contributed to declines in new diagnoses and deaths from CRC. Nevertheless, for the State to prevent and detect more CRC cases and pre-cancerous polyps, there still needs to be upscaling of resources and workflows to increase screening with evidence-based interventions at primary care clinics. Many of Washington’s most vulnerable populations receive care from Washington’s Federally Qualified Health Centers (FQHCs). According to UDS (Uniform Data System) reports, only 48.5% of FQHC patients ages 50-75 were up to date with recommended CRC screening in 2018, which is far below the state rate of 72.2%. The majority of patients (88.6%) are living at or below 200% of federal poverty level (FPL), over half (57.3%) are covered by Medicaid, and one sixth remain uninsured (16.6%). Over half (55%) of FQHC patients represent racial and ethnic minority groups. Beyond uninsured and underinsured populations who live at or below 250% FPL, Hispanic, American Indian, and Alaska Native populations are demographic groups that experience a greater burden of colorectal cancer and BCCHP will prioritize partnerships and increase access to these groups across the state. According to the 2018 BRFSS, 53.8% Hispanic and 65.7% of American Indian and Alaska Native (AI/AN) were up to date compared to 72.2% statewide. The Washington State Cancer Registry reported that in 2017 56.3% and 55.6% of colorectal cancer cases were diagnosed at late stage, for AI/AN and Hispanic residents respectively. We will expand partnerships with FQHCs and Tribal and Rural clinics that see disparities in screening. We will start with six FQHC partners implementing EBIs and work to bring on an additional three rural or tribal clinics in year one. We will expand partnerships each year. Outcomes are: •Establish and maintain health system and community partnerships that support increased Colorectal cancer screening and complete readiness assessments with each partnering Federally Qualified Health Centers (FQHC) system and participating clinic level sites; •Increase the number of EBIs for CRC screening in place among FQHCs and enhance the quality of implementation of EBIs in partnering FQHCs; •Increase clinic-level CRC screening rates among partnering clinics; and facilitate linkages to follow-up colonoscopy after an abnormal screening test. •Increase number of colorectal cancers prevented and diagnosed at an early-stage;
