Background: Colorectal cancer (CRC) is a significant burden and health disparity among Alaska Native people who have the highest recorded CRC incidence and death rates in the world; twofold higher than among U.S. Whites. Alaska Native CRC screening rates are slowly increasing, but remain lower than other populations. CRC screening rates are especially low in Alaska’s small remote communities, which are not connected by road to the rest of the state. The Alaska Native Tribal Health Consortium (ANTHC) is a statewide, tribally owned and operated, non-profit health organization providing services to members of the 229 federally recognized Alaska Native tribes. Purpose: The ANTHC CRC Control Program (CRCCP) is applying for CDC-RFA-DP20-2002 funding to improve CRC screening among Alaska Native people ages 50-75 living in frontier and remote areas of Alaska. ANTHC will implement this program by partnering with six regional Tribal health systems and the 138 community clinics that they operate located in western, interior, and southeast Alaska. The ANTHC CRCCP will expand use of multicomponent culturally adapted evidence-based interventions (EBIs) as well as health information technology systems improvements, to address barriers to screening and improve Tribal health systems of care. Outcomes: By the end of the 5-year project period, ANTHC will achieve the short-term outcomes of increased formal partnerships that support screening; adoption of quality CRC screening standards and workflows; improved Tribal health systems data; increased patient and provider adherence to screening guidelines and standards; increased EBI adoption at Tribal health systems; reduced Alaska Native patient barriers to CRC screening; increased number of policies/system changes to support access to high-quality screening; and increased Alaska Native CRC screening rates. By the end of the project period ANTHC will achieve the intermediate outcomes of increased number of cancers prevented; increased detection of early-stage CRC; and decreased disparities in Alaska Native CRC screening outcomes. Through this funding ANTHC will also make progress toward long-term outcomes, including reduced CRC incidence and mortality among Alaska Native people. ANTHC has worked over the past 20 years as a leader in CRC prevention among Alaska Native people and establishing best practices for reaching rural/remote Tribal populations. ANTHC participated in the CDC-funded CRCCP from 2009-2015, which led to a 68% increase in screening among Alaska Native people served by Tribal health systems participating in the ANTHC-led program. ANTHC has the capacity and extensive experience to ensure successful implementation of this program, through which we will substantially improve CRC screening rates and implement sustainable clinical practices. This will help accomplish our ultimate goal to decrease CRC and achieve our vision: Alaska Native people are the healthiest people in the world.
