The Iowa Department of Public Health (IDPH) proposes to raise colorectal cancer screening rates in Iowa’s Federally Qualified Health Centers (FQHCs) to 60 percent by 2025 through strategies proposed in this application entitled “Public Health and Health Systems Partnerships to Increase Colorectal Cancer Screenings in Clinical Settings.” This project will be accomplished by facilitating implementation of evidence-based interventions in 10 of Iowa’s FQHCs, which include : 1) All Care Health Center; 2) Community Health Care, Inc.; 3) Community Health Center of Fort Dodge; 4) Community Health Centers of Southeastern Iowa; 5) Community Health Centers of Southern Iowa; 6) Crescent Community Health Center; 7) Eastern Iowa Health Center; 8) Primary Health Care, Inc.; 9) Promise Community Health Center; and 10) River Hills Community Health Center. All of the identified FQHCs have colorectal cancer screening rates below 60 percent. In the last year, the group of FQHCs served a population of more than 30,500 Iowans ages 50 -75 across the state. This population represents Iowa’s most vulnerable citizens, including those who are underserved, uninsured, and who have an average risk of colorectal cancer. This is the project’s target population A key partner supporting and providing technical assistance to the FQHCs is the Iowa Primary Care Association (Iowa PCA). Other key partners to be engaged beginning in Year 1, and continuing through the five-year project period include the American Cancer Society (ACS), Iowa Cancer Consortium, Cancer Prevention and Control Network, Colon Cancer Foundation of Iowa and IDPH chronic disease programs. The implementation of EBIs chosen by the Centers for Disease Control and Prevention as the key intervention for raising colorectal cancer screening rates employs strategies recommended by The Community Guide. IDPH will implement required strategies with technical assistance and guidance provided by the Iowa PCA. Iowa PCA has a proven, successful history of raising colorectal cancer screening rates in Iowa’s FQHCs. The project structure includes a needs assessment and implementation of at least two EBIs per FQHC to raise colorectal cancer screening rates in ten FQHCs. The project has defined activities under the following six strategies that will support achievement of expected outcomes: (1) Establish Partnerships with Health Systems; (2) Establish Partnerships to Support Implementation of evidence-based interventions (EBIs); (3) Conduct Implementation Readiness Assessment of Partner Primary Care; (4) Implement Evidence-Based Interventions; (5) Facilitate Patients’ Linkage to Follow-Up Colonoscopy; and (6) Data Quality, Program Monitoring and Evaluation. Performance in implementing these strategies will be monitored and evaluated. This project will increase colorectal cancer screening rates in Iowa FQHCs, as well as decrease disparities in colorectal cancer incidence and mortality.
