Young breast cancer survivors (YBCS) face myriad challenges related to their health, psychosocial needs, finances, and other barriers to care. These challenges are pronounced in young women with metastatic breast cancer (mBC), as well as underserved populations such as ethnic or linguistic minorities, low-socioeconomic groups, un/underinsured populations, women with disabilities, and lesbian, gay, bisexual, and transgender (LGBT) communities. Texas’ diverse population, state characteristics, and health resource limitations increase the likelihood of suboptimal survivorship care. Purpose The project will develop an integrated response to optimize YBCS/mBC survivorship care in Texas. The Breast Cancer Resource Center (BCRC) is a nonprofit organization well-suited to facilitate multiple levels of change which improve health and quality of life for YBCS/mBC and their families. BCRC will coordinate and mobilize stakeholders to develop strategic, actionable, and measurable population-based activities. This approach is consistent with framework of the Public Health Action Model for Cancer Survivorship. Outcomes Project design generally includes: 1) creation of a stakeholder network and multidisciplinary partnerships; 2) enhanced YBCS/mBC data collection methods including needs assessment and focus groups; 3) expansion of informed, integrated care services for YBCS/mBC, including health system and community interventions; and 4) education and development of shared-care best practices. The project will result in multidisciplinary recommendations which are scalable and replicable for similar communities, including those lacking comprehensive cancer care. Anticipated five-year outcomes include but are not limited to: • Increased awareness of strategies to mitigate health risks and promote healthy lifestyles among cancer survivors and caregivers; • Increased awareness among health care providers on the needs of cancer survivors; • Increased access to emotional/psychosocial support systems among cancer survivors; • Increased access to lifestyle programs, clinical preventive services, and follow-up cancer care for cancer survivors; • Increased use of lifestyle programs, clinical preventive services, and cancer care among cancer survivors; and • Improved quality of life among YBCS.
