The Department of Health (DOH) is the lead agency for the statewide prevention, control and management of chronic diseases in Puerto Rico. The Division of Chronic Disease Prevention and Control’s (DCD) efforts are based on those practices shown to be successful and recommended in Best Practices from various guidelines compiled by The Centers for Disease Control and Prevention (CDC) and at the local level. The strategies, objectives and activities presented in this document reflect the contribution of internal and external partners and focuses on evidence-based approaches for reducing risk factors associates to Diabetes and Cardio Vascular Disease (tobacco, hypertension, cholesterol, among others) while at the same time providing guidance in policy development and professional development among the healthcare workforce. While the activities intend to address all populations, special emphasis is placed on adults older (65+) with low income (Diabetes, hypertension and obesity/overweight) and those among the 45 to 54 age group with government health insurance (tobacco use). All these efforts, as well as those from the rest of the component in this application will be measured and monitored a developing chronic disease surveillance system.
