Prevention and Control of Chronic Disease and Associated Risk Factors in the U.S. Affiliated Pacific Islands, U.S. Virgin Islands and Puerto Rico CDC-RFA-DP19-1901 - Application PROJECT ABSTRACT SUMMARY The Virgin Islands Department of Health in conjunction with its partners intends to use the funding in this NOFO, if granted, to equip the community with knowledge and engage the community in behaviors that will ultimately reduce the prevalence of diabetes, prediabetes, tobacco use and cardiovascular disease. The funding will also allow for collaboration with the providers who are deemed with prescribing the most updated standard of practice when caring for the population with diabetes, prediabetes, heart disease and chronic tobacco use. Targeting the risk factors such as obesity, lack of physical activity, depression, increased stress and the unhealthy eating will be the objectives of programs instituted. This will be done through education and monitoring of providers’ and healthcare extenders’ practices, community education and awareness activities. This document will showcase a work plan will describe in detail the approaches and strategies that will be utilized to achieve the success of a continuous program. The work plan strategies and objectives will be used through the years of this collaborative. The collaborative will utilize strategies that focus on the following: 1. Systematically collecting, sharing and using data to guide the decision making, health care program implemented and evaluation of the programs’ success. 2. Implementation of environmentally driven programs that address tobacco use, eating habits and physical activity. 3. Improvement in the quality of clinical care as it relates to preventing chronic disease 4. Improving access to, increasing participation in and improving awareness of prevention and disease management programs. To implement the strategies, we will have to build capacity for the work and must develop positive relationships with community-based organizations, governmental agencies, faith-based organizations and agencies that are in the business of providing health care and health care support. Evaluation of the strategies implemented will be conducted by the introduction of partners, such as Caribbean Research Center (CERC) of the University of the Virgin Island and Centers for Disease Control and Prevention (CDC) that will advise and direct the evaluation process. Collaborations with stakeholders and partners are essential to the development, approval and implementation of the territorial strategic plan and the evaluation plan. The entire chronic disease prevention program components of this NOFO will be guided by the chronic disease advisory group. The current non-communicable disease territorial plan is under revision and will be finalized within the first two years of this NOFO.
