Purpose: One of Southcentral Foundation’s (SCF) corporate objectives is to “Reduce the rate of and improve the management of cardiovascular disease.” Acting in alignment with SCF’s mission of, “Working together with the Native Community to achieve wellness through health and related services,” the SCF WISEWOMAN (WW) Program will facilitate partnerships between SCF’s Primary Care Centers (PCCs), SCF’s Health Education Departments, Physical Therapy and Exercise (PTE) and other SCF chronic disease programs, and external organizations to provide Alaska Native and American Indian (AN/AI)women, ages 40 to 64, with healthy behavior support services (HBSS) along with education and the tools they need to reduce or control their cardiovascular disease (CVD) risk factors. SCF will build upon the significant progress made in the health coaching process in the coming next five years. It is estimated that a minimum of 2,250 women will receive WISEWOMAN services in the first fiscal year and at least 1800 of those women will receive HBSS. We expect these number to continue during grant years two through five reaching a minimum total of 11,250 women for five years. Outcomes: By the end of the proposed program period, SCF anticipates a decrease in the number of modifiable risk factors that AN/AI women living in Anchorage and the Matanuska-Susitna (Mat-Su) Valley have. SCF will ensure this objective is met by working to accomplish the following outcomes: 1) Increase control of high blood pressure in WW participants; 2) Environmental changes in the AN/AI communities that result in an increase in: places for physical activity, access to healthy food, access to smoking cessation programs, and smoke free public places; 3) Increase in appropriate screenings and services provided to participants through the WW Program; and, 4) Implementation of policies that focus on primary prevention and follow national guidelines. SCF WW Program: SCF has been awarded a WW grant for the past 18 years and has been a NBCCEDP grantee for the past 25 years. The SCF WISEWOMAN Program is fully integrated with the NBCCED Program. The Program Director, Data Manager and Program Evaluator are key staff and all serve in the same role on both programs. SCF has collaborative and strategic partnerships within SCF departments at the Anchorage Native Health Campus that includes Health Education (HED), Physical Therapy & Exercise (PT&E), Registered Dietitians (RDs), Data Services, Information Technology, Finance, Organizational Development, and seven Primary Care Clinics(PCC). The Valley Native PCC includes PT&E, HED and 2 PCC's. Core Component: SCF’s WW Program will focus on AN/AI women, ages 40 to 64, in SCF’s primary service area, which includes the Municipality of Anchorage and the adjacent Mat-Su. Alaska Natives in the proposed service area have a quality of life below national health and income standards. Poverty is universally recognized as a risk factor – Anchorage Native people earn 37 percent less income, and experience nearly three times the rates of poverty than the rest of the Anchorage population. Innovation Component: SCF plans to use Innovation Component funds to expand WW services for AN/AI women, ages 40 to 64, at 12 Federally Qualified Health Center’s (FQHC’s) in rural south central Alaska. At the request of local tribal governments, SCF assumed management of health care services at the following 12 sparsely populated FQHC’s: Nilavena Subregional Clinic, McGrath Health Center, Takotna Clinic, Nikolai Clinic, C’eyiits’ Hwnax Life House Rural Community Health Center, Indian Creek Health Center and Port Alsworth Health Center, St. Paul Health Center, Kokhanok Clinic, Nondalton Clinic, Igiugig Village Health Clinic and Pedro Bay Clinic. SCF's Detox Center in Anchorage is the latest FQHC. The FQHCs are federally-recognized Medically Undeserved Areas and designated as Health Professions Shortage Areas.
