The Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes and Heart Disease and Stroke grant will be utilized to support novel approaches to address cardiovascular disease (CVD) and diabetes within populations and communities that are disproportionately affected in Florida. Florida is a vast and diverse state with over 20.2 million residents in 67 counties. In Florida, heart disease accounts for 3 out of every 10 deaths while diabetes is the 7th leading cause of death. The highest prevalence of these chronic diseases is found in the most rural counties which accounts for nearly 70% of Florida land. The department seeks to create, pilot and evaluate innovative strategies to increase the use of evidence based practices that will ultimately reduce the burden of diabetes and CVD in the state. With an emphasis on health equity, new relationships with nontraditional partners will be developed with those who serves populations most at risk such as the elderly, Native Americans, and persons with disabilities. The bureau will expand current partnerships with community organizations including local health departments and identify ways to eliminate barriers to participation and retention in CDC recognized lifestyle change programs and/or ADA recognized/AADE-accredited diabetes self-management education and support (DSMES) programs. These efforts will focus on novel approaches such as teleservices to maximize reach to high burden and underserved populations. Collaborations with Community Health Workers (CHW) and health care extenders will be fostered as a means of strengthening the important link between community resources and clinical services. The bureau will also partner with health systems and non-physician providers to explore the adoption of mechanisms that promote early detection of chronic conditions, medication therapy management (MTMS) and overall improvement of care. The bureau will make an effort to ensure CVD and d iabetes efforts will work collaboratively as to give rise to a synergistic impact on populations that are at risk or impacted by both conditions. The bureau will continually analyze successes and barriers through data evaluation and conduct quality improvement measures to ensure optimal outcomes throughout the life span of this funding.
