Background: Public Health Services (PHS) is within the County of San Diego (COSD) Health and Human Services Agency (HHSA) and serves as the local health department for the entire San Diego region. Through the COSD’s Live Well San Diego long-term vision to promote healthy, safe, and thriving communities, HHSA is committed to prevent and reduce chronic disease through implementing lifestyle, health systems, and community-clinical linkage change. HHSA will lead implementation of Healthy Works: Innovative Public Health Strategies to Prevent and Manage Diabetes and Heart Disease and Stroke. Purpose: To build on successes and lessons learned from CDC-RFA-DP14-1422 to design, test, and evaluate innovative approaches to address significant health disparities, specifically in the Hispanic/Latino and African-American communities, to reduce risks, complications, and barriers to prevention and control of diabetes and cardiovascular disease among San Diego’s high-burdened populations. Expected Outcomes: The County of San Diego will reach long-term goals (increased number of people with prediabetes enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss, decreased proportion of people with diabetes with an AIC>9, increased control among adults with known high blood pressure, and increased cholesterol management among patients at high risk for cardiovascular events) by achieving the following intermediate outcomes over the project period: - Increased enrollment and retention in CDC-recognized organizations delivering the National Diabetes Prevention Program lifestyle change program; - Increased number of people with diabetes served by healthcare systems actively using clinical systems and care practices to improve diabetes-related health outcomes; - Increased medication adherence among patients with high blood cholesterol; - Increased engagement in self-management among patients with high blood pressure and high blood cholesterol; - Increased participated in evidence-based lifestyle interventions among patients with high blood pressure and high blood cholesterol. Conclusion: Given the diverse demographics and health disparities for diabetes, heart disease, and stroke in the County of San Diego, as well as the rich history of established partnership and the capacity to leverage accomplishments achieved in the healthcare systems and community-clinical linkage domains of CDC-RFA-DP14-1422, HHSA is well-positioned to implement this proposal.
