The Philly Difference: Connections for Better Chronic Care, Part 2 is organized around empowerment of the patient. Our integrated work plan for Categories A and B engages our partners in giving patients crucial tools and support for prevention and self-management of prediabetes, diabetes, hypertension and high cholesterol. With the help of our partners, we will: • Spread DPP and DSME to populations in the city who are at greater risk for prediabetes and diabetes; • Assist patients with opportunities for behavioral health support in overcoming their internal barriers to prevention and control of their chronic condition(s); • Create and implement a campaign targeted at insurers and self-insuring businesses to “Cover the Cuff,” so that more patients can collaborate with their providers in self-monitoring their blood pressure and controlling their hypertension; • Provide patients with opportunities on-site and through telehealth for earlier detection of conditions that can impair independent functioning and quality of life (diabetic retinopathy and early CKD screening). • Create bi-directional networks that (1) enable effective pharmacist-provider-patient communication for better management and medication adherence for chronic conditions, and (2) bridge patient supports from the clinical setting into the community. Successful management and treatment of prediabetes, diabetes, hypertension and high cholesterol can’t happen without patient participation. Our project aims to make patients part of the care team.
