Louisiana Actions to Control Diabetes Plus (WAL CD+) will use statewide and regional innovative strategies to enhance access to evidence-based programs, improve patient care processes, enhance community-clinical linkages, improve patient referral mechanisms, and improve the tracking and use of clinical data to control and reduce the burden of diabetes in Louisiana’s high-burden populations and statewide. Well-Ahead Louisiana (WAL) proposes to leverage our strong relationships with regional partners to design, test and evaluate innovative approaches to diabetes prevention and management. WAL Diabetes Prevention and Management programs have undertaken initiatives in partnership with health systems, local community partners, and other state offices. The WAL CD+ program will build upon the strengths of these relationships and enhance this work by leveraging Health Information Technology resources. WAL CD+ activities will be mutually reinforcing in the parishes selected for other WAL diabetes prevention and management strategies. The innovative resources offered within this grant will allow health systems to take a next step to increase their ability to offer optimal care for chronic disease management. WAL proposes to utilize these innovative technology solutions to improve provider education on best-practices and chronic care guidelines, increase the number of individuals receiving optimal care for diabetes, and increase access to lifestyle change programs. WAL expects to achieve the following outcomes related to type 2 diabetes prevention and diabetes management: Type 2 Diabetes Prevention: 1) Increased access to the National DPP lifestyle change program in underserved areas, such as low-income HPSA designated parishes, for people with diabetes; (2) Increased community clinical links between healthcare systems and local NDPP programs that support bidirectional referrals to assist in enrolling and retaining participants in the National DPP lifestyle change program. (3) Increased enrollment and retention in CDC-recognized organizations delivering the National DPP lifestyle change program in Louisiana; (4) Increased number of people with prediabetes in Louisiana enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss. Diabetes Management: (5) Increased adoption and use of clinical systems and care practices, such as Project Echo and telemedicine, to improve health outcomes for people with diabetes. (6) Increased number of people with diabetes served by healthcare systems actively using clinical systems and care practices, such as Project Echo and telemedicine, to improve diabetes-related health outcomes;(7) Decreased portion of people with diabetes in Louisiana with an A1C>9.
