The Southern Maryland Chronic Disease Prevention Program (SMCDPP) will implement diabetes, heart disease and stroke prevention interventions in a four (4) county area (Prince George’s, Calvert, Charles, and St. Mary’s) in Southern Maryland. The target area for this project has a total population of 1,276,625 residents and comprises urban, suburban and rural communities. The project will target all target area residents at risk for or diagnosed with Type 2 diabetes, heart disease and/or stroke. SMCDPP priority populations are patients who are high utilizers of health services estimated to be 30,000 in number; patients with chronic disease who are racial/ethnic minorities, estimated population size 345,000; and patients who are rural residents, estimated as 157,000 in number. SMCDPP seeks to build a primary care network and community of practice for chronic care (Type 2 diabetes and cardiovascular disease (CVD)) patients in Southern Maryland, with a focus on those who are high service utilizers. The program will bring together contiguous counties in a collaborative that seeks to build on prior health equity initiatives in the region by expanding the integration of public health and clinical practice to improve participation and retention in National Diabetes Prevention Program (DPP) interventions; improve prevention and clinical management of Type 2 diabetes; track and monitor clinical measures shown to improve healthcare quality for patients with CVD or at risk for CVD; implement team-based care for patients with CVD; and coordinate bi-directional referrals for chronic disease patients to community resources, clinical services, and interventions promoting self-management, and lifestyle change for patients with CVD/ history of a cardiac event. The expected outcome of the project are: • increased access to diabetes, CVD, and stroke prevention and treatment resources for the population in general and priority populations – high utilizers, racial/ethnic minorities, and rural residents; • associated improvements in patient knowledge ( understanding of clinical measures such as blood pressure reading) and patient engagement in self-management; clinical measures (HbA1c, blood pressure, cholesterol readings); • improved quality of diabetes, CVD, and stroke prevention and treatment as evidenced by provider uptake of clinical decision support tools, national practice guidelines, integrated workflows, team-based care, and use of non-clinical providers such as community health workers; • improved infrastructure to deliver quality diabetes, CVD, and stroke prevention and treatment services as evidenced by increased numbers of evidence-based lifestyle coaching services; increased payer reimbursement for lifestyle coaching services; prevention and treatment services provided by telehealth; and a Community of Practice devoted to identifying and disseminating best practices, lessons learned, and innovations in diabetes, CVD, and stroke prevention and treatment in the region; • and decreased healthcare costs associated with the chronic disease care and treatment
