Prediabetes and diabetes are leading public health problems in Chicago. Chicago, the third largest city in the nation, with a population of 2.7 million, has a diabetes prevalence of 10.6%, disproportionately affecting racial and ethnic minorities and lower socioeconomic status. Applying CDC estimates, at a minimum, Chicago has 695,000 people with prediabetes. This is likely an underestimate, given Chicago’s higher proportion of minorities and low-income individuals at greatest risk for prediabetes. Despite the strong evidence and community-based translation of the Diabetes Prevention Program (DPP), Chicago is a “DPP desert” with limited access to CDC-recognized lifestyle change programs (CDC-RLCPs). Chicago is poised for transformation. The Chicago Department of Public Health, in collaboration with its bona fide agent the Illinois Public Health Institute, proposes to implement the Chicago Collaboration to Advance Reach, Equity, and Systems to Prevent Diabetes (Chicago CARES). Chicago CARES aims to reduce diabetes disparities and promote health equity by launching a Network of CDC-RLCPs and a learning collaborative to scale teleretinal diabetic retinopathy screening in high-burden, underserved communities. As a hub for resources, infrastructure and technical expertise, the Chicago CARES Network will support the development, expansion and sustainability of CDC-RLCPs by 1) developing a coordinated bi-directional e-referral system; 2) fostering cross-program collaboration to enhance efficacy and quality of program operations and outcomes; and 3) reducing barriers to participation and retention with a focus on building sustainability and payer engagement to advance coverage of the program. The Chicago CARES’ target populations are low-socioeconomic status, African Americans and Hispanics/Latinos, 20 years and older. The goal of the Chicago CARES Network is to, by 2023, achieve access to a CDC-RLCP in all 53 Chicago community areas rated “high” or “medium” on Chicago’s “hardship index,” a measure of neighborhood-level socioeconomic conditions. The Chicago CARES collaboration is comprised of nearly two dozen partners representing leading safety-net health care providers and provider networks, CHW leaders, health IT, practice transformation experts, community organizations, Medicaid Managed Care Organizations, nationally recognized diabetes prevention researchers and national organizations that are key partners in the National Diabetes Prevention Program. Chicago CARES will carry out six strategies: implement a bi-directional e-referral system; increase access to CDC-RLCP including a community-based CHW delivery model; providing advanced training to continuously enhance program quality; conduct robust, culturally tailored grassroots messaging and communications; innovate to increase participation and retention through building on current momentum for Medicaid managed care coverage and a third-party organization for permanent infrastructure and linking to wrap around resources like fitness waivers, food access, and other social determinants of health; and support health care practices in implementing teleretinal diabetic retinopathy screening. Chicago CARES will use the RE-AIM evaluation framework to ensure process and outcome evaluation at the systems, organizational, and individual level. Key evaluation questions focus on the extent to which the strategies improved health outcomes, increased knowledge about prediabetes and influenced intent to change health behaviors, created systems and infrastructure to support referrals, recruitment and sustainability of CDC-RCLP, and promoted quality and fidelity. Through the extensive groundwork already laid and a strong collaboration of multi-sector, multi-disciplinary partners, Chicago CARES is well positioned to scale and sustain CDC-RLCPs and strengthen diabetes care systems in communities that need it the most.
