Led by the Nevada Division of Public and Behavioral Health (DPBH) Bureau of Child, Family and Community Wellness (CFCW), Chronic Disease Prevention and Health Promotion (CDPHP) Section, the goal of this five-year cooperative agreement application is to design, test, and evaluate innovative approaches to reduce the burden of diabetes and cardiovascular disease (CVD) in high-burden populations within Nevada. Target populations will include residents who are living with or at-risk for cardiovascular disease and diabetes with special emphasis on Hispanics, African Americans, low-income populations, older adults, uninsured individuals, and rural and frontier residents, as well as populations living in underserved areas of urban counties. In Category A, the CDPHP Section aims to improve access to, and participation and retention in, the National Diabetes Prevention Program (National DPP) through four (4) strategies: A.1. Implement systems to facilitate bi-directional e-referral between healthcare systems and the National DPP; A.2. Support organizations to increase enrollment in existing National DPP or establish and sustain new National DPP in underserved areas; A.3. Implement tailored messaging to reach underserved populations at-risk for type 2 diabetes to increase awareness of prediabetes and the National DPP; and A.4. Support advanced training for lifestyle coaches working at the National DPP to strengthen skills needed for engagement and retention of participants. To help prevent and/or manage type 2 diabetes, CDPHP will work with statewide partners to implement two (2) strategies: A.5. Explore and test innovative ways of eliminating barriers for participation and retention in the National DPP for type 2 diabetes prevention and/or ADA-recognized/AADE-accredited diabetes self-management education and support programs for diabetes management among high burden populations, and A.6. Work with healthcare systems to expand the use of telehealth technology for increasing access to the National DPP for type 2 diabetes prevention in underserved areas. In Category B, which focuses on prevention and management of CVD, CDPHP will implement the following strategy: B.1. Increase identification of patients with undiagnosed hypertension using EHRs. CDPHP will promote team-based care for patients with high blood pressure and high blood cholesterol through two (2) strategies: B.3. Explore and test innovative ways to engage non-physician team members in hypertension and cholesterol management in the clinical setting, and B.4. Promote the adoption of MTM between community pharmacists and physicians for the management of high blood pressure, high blood cholesterol, and lifestyle modifications. Additionally, CDPHP will link community resources and clinical services by implementing three (3) strategies: B.5. Facilitate the engagement of community health workers in hypertension and cholesterol management in clinical and community settings; B.6. Implement systems to facilitate bi-directional referrals between community programs/resources and healthcare systems; and B.7. Explore and test innovative ways to expand the use of mobile health technology to promote hypertension and cholesterol management. To evaluate both Categories A and B strategies/activities, CDPHP will forge sustainable, statewide multi-sector partnerships with critical stakeholders. Nevada’s long-term project outcomes include: 1) decreased proportion of Nevadans with diabetes who have an A1c > 9; 2) increased number of targeted population with prediabetes enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss; 3) increased control among adults with known high blood pressure; and 4) increased cholesterol management.
