Georgia Department of Public Health (DPH) seeks funding under CDC-RFA-DP18-1815, to continue implementation and evaluation of evidence-based strategies to prevent and manage cardiovascular disease (CVD) and diabetes in high burden populations/communities within the state. Georgia is poised to make a difference in the state through this funding announcement because promoting and maintaining population health has always required combined effort from public health offices, private healthcare entities, and social service organizations. In this opportunity state and local public health will partner with large health systems, emergency medical services as well as community-based organizations. Each sector plays its own distinct role in addressing the community, individual, and social factors that influence health and wellness. Many of the most severe health challenges that Georgians face today are closely related to the conditions in which individuals live, work, and play, including costly chronic diseases such as diabetes, heart disease and stroke. To truly improve population health at scale, public health offices and private healthcare organizations cannot operate alone. Collaborations between the public and private sectors, as well as community-level organizations, will catalyze effective, coordinated care, and confront the socioeconomic conditions that can lead to poor health. Georgia is requesting funding to implement six of the strategies outlined in the funding announcement simultaneously in selected communities for both category A and category B and has submitted one uniform application to support funding under one or both categories. Coinciding with the State Health Improvement Plan’s goals of decreasing the annual rate of hospitalizations for diabetes by 25 percent and for hypertension by 10 percent, Georgia will achieve the following outcomes as a result of implementation of the strategies: (1) decreased proportion of Georgians with diabetes with an A1C>9; (2) increased number of people with prediabetes that are enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss; and (3) increased control among Georgia adults with known high blood pressure and high blood cholesterol. In accordance with CDC-RFA-DP18-1817, Georgia’s proposal as presented focuses on addressing the burden of cardiovascular disease through the following Healthy People 2020 goals (HDS-3, HDS-4; HDS-7, HDS-8, HDS-12, D-14, D-16.1, D-16.2, and D-16.3).
