Two leading causes of mortality in New York City (NYC) include cardiovascular disease (CVD; 200 deaths per 100,000 adults) and diabetes mellitus (DM; 21.7 deaths per 100,000). DM and high blood pressure (HBP) disproportionally affect the city’s low-income residents and people of color. While the citywide prevalence in 2017 for hypertension (HTN) is 28.0%, it is 35.1% among blacks and 32.6% for Latinos, and lower in whites (23.1%); DM prevalence is 11.5% citywide compared to 15.0% for blacks, 15.7% among Latinos, and 6.9% in whites. Stark disparities by geography also justify concentrating resources in areas of highest need. Death before age 65 attributed to DM in East Tremont is 14.1 per 100,000 residents compared to 2.1 in the Upper West Side of Manhattan. Adjusted prevalence for HTN is 38.2% in Northeast Bronx compared with 20.2% in Upper West Side, Manhattan. The NYC Department of Health and Mental Hygiene (DOHMH) and its bona fide fiduciary agent, the Fund for Public Health in NYC request funding for “Take Action ABCD,” an initiative to improve A1C control, Blood pressure control, Cholesterol management and Diabetes prevention and management for adult residents of NYC. With twelve complementary strategies, the initiative aims to improve health for adults for whom pre-diabetes, DM, HBP, and high blood cholesterol (HBC) prevalence is higher than the city average and supplements activities in neighborhoods for a priority population disproportionately affected by CVD and DM due to historical and contemporary discrimination. Collectively, the selected neighborhoods have nearly 4 million adult residents with 530,000 persons with DM and nearly 1.3 million with HTN. From electronic health record (EHR) data representing 500+ primary care practices engaged in DOHMH programs, an estimated 91,175 persons meet criteria for prediabetes and 100,285 have HBC. For those who had at least one health care visit, 15,260 have A1C levels >9 (poor A1C control rate of 19%), while only 68% of those with or at risk for CVD have been prescribed statin therapy and 66% of those with HTN are considered controlled. Further, only 63% are adherent to their HTN medications and 58% are adherent to the HBC medications. We will partner with 400 health care sites and 175 independently owned pharmacies to promote proactive health care teams by offering practice facilitation, actionable data reports, trainings to providers and non-physician staff on EHR systems, technical assistance with bi-directional electronic referral system, and connections between clinical and community resources to activate individuals for health. Place-based activities for neighborhoods with the highest disparities include: partnering with 45+ community-based organizations and clinical systems to increase accessibility and effectiveness of lifestyle change programs; tailoring communications to raise awareness of risks and resources; training community health workers to strengthen the workforce and support service delivery; and enhancing bi-directional referral resources to improve linkages to services that impact health outcomes. Through NYC Take Action ABCD, DOHMH anticipates leveraging over 600 partnerships and will: increase access to National DPP for 16,575 individuals; increase the proportion of people with prediabetes enrolled in the National DPP who achieve 5-7% weight loss by 27% (22% to 28%); improve care and management for 115,000 people with DM and 330,000 with HBP; increase medication adherence for HBP by 11% (63% to 70%) and HBC by 9% (58% to 63%); decrease the proportion of people with DM with an A1C > 9 by 16% (19% to 16%); increase BP control among people with HBP by 6% (66% to 70%); and improve cholesterol management among people with HBC by 3% (68% to 70%). Using robust data collection, program evaluation, and reporting processes, Take Action will share and disseminate lessons learned as well as monitor progress in reducing morbidity and mortality from CVD and DM.
