Virginia faces a public health crisis, as diabetes and cardiovascular disease are rampant across communities, particularly in underserved areas less equipped to address these diseases and conditions. Nearly 5.8 million Virginians suffer from prediabetes, diabetes, high blood pressure, high blood cholesterol, and cardiovascular disease. Lack of access to healthcare services and community-based programs is a major factor that creates the conditions that further complicate and prolong the mitigation of these chronic diseases. The Virginia Department of Health (VDH) will collaborate with partners to design, test, and evaluate innovative approaches for the delivery of clinical and community-based interventions that improve the prevention and control of diabetes and cardiovascular disease in underserved areas. The VDH collaborates with key stakeholders who altogether provide mutually-reinforcing resources to ensure the successful implementation of InnoVAte, Virginia’s innovation framework strategically coordinating partners at the state, health system, and community level. At the state level, the Virginia Hospital and Healthcare Association (VHHA) provides access to real-time healthcare system and hospital data, utilizes predictive modeling to identify target health systems, priority patients, and hot spot communities, and allows for rapid scaling of InnoVAte across Virginia’s health systems. The University of Virginia (UVA) Health System’s Center for Telehealth brings statewide capacity for the development, expansion, and innovation of telehealth technology and broadband access, testing of National Diabetes Prevention Program and cardiac rehabilitation programs, and development of an online training portal and toolkit of health system interventions and tools. At the health system level, UVA and VDH will partner with health systems to test team-based care and telehealth delivery models. UVA will also collaborate with electronic health records to create bi-directional referral systems to be used by health systems, primary care practices, and Federally Qualified Health Centers. Virginia Commonwealth University Health System’s Center for Pharmacy Practice Innovation and Center for Telehealth will implement activities to prevent and control diabetes and cardiovascular disease in a localized approach while exchanging best practices with UVA. Simultaneously, community level partners who will build capacity for community-based programs include the YMCA, community pharmacies through the Virginia Pharmacists Association, and the communities with established networks of clinical and community partnerships through the CDC DP13-1305 and CDC DP14-1422 grants. Key partners will work to align resources, services, and expertise to reach target patient populations using predictive modeling, telehealth services, cutting-edge technology, clinical decision supports, interprofessional team-based models, and bi-directional referral systems to community programs. Over the 5-year period, VDH will scale and sustain successful and comprehensive interventions to 5-7 health systems and underserved areas by 2023. Through leveraging existing resources at state and local levels, building capacity in key stakeholders at the community level, and coordinating clinical and community interventions in a mutually-reinforcing approach, InnoVAte provides the public health response critical for mitigating the widespread diabetes and cardiovascular disease-related disease and conditions in Virginia.
