The Chronic Disease Prevention & Management Strategy: Innovative Solutions for Healthier Communities (CDPMS) will unite the Los Angeles County Department of Public Health, the Los Angeles County Department of Health Services, local federally qualified health centers, and community-focused partners to improve the health of adults in Los Angeles County. This jurisdiction is home to over 10 million residents, and has 88 incorporated cities and a large unincorporated area with approximately 140 communities. CDPMS will build upon the momentum of the 2014-2018 Chronic Disease Prevention Strategy (CDC 1422 initiative) to implement a multi-pronged, comprehensive model of chronic disease prevention and control. The effort will employ evidence-based strategies and innovative approaches to better prevent and manage diabetes and cardiovascular disease in Los Angeles County. Through institutional policies, organizational practices, trainings and technical assistance, and leveraging of key interventions (e.g., telehealth, culturally/linguistically tailored communication/messaging for target audiences, use of pharmacists and community health workers), CDPMS intends to scale programs and disseminate best practices that have proven efficacy and impact. These practices and programs include the National Diabetes Prevention Program, diabetes self-management education, team care, patient health navigation and coaching, and comprehensive medication management. CDPMS will accomplish these goals by executing the 16 Category A and Category B strategies outlined in the DP18-1817 Notice of Funding Opportunity. They will be implemented simultaneously and synergistically to create an integrated, countywide chronic disease prevention and management structure. The structure will function across clinic and community settings to maximize reach and impact for groups experiencing disproportionately high burden of chronic disease. By project end, these mutually reinforcing strategies should result in a marked reduction of prediabetes, type 2 diabetes, hypertension, high cholesterol, and other cardiovascular disease conditions in Los Angeles County’s high burden populations: those with low-socioeconomic status, Latinos, Asians/Pacific Islanders, and African American men.
