RI Department of Health’s Diabetes, Heart Disease, and Stroke (RIDHDS) Program will utilize funding opportunity CDC-RFA-DP18-1817 to build upon the accomplishments achieved in DP13-1305 and DP14-1422, and to expand the reach and impact of anticipated DP18-1815 activities by both extending evidence-based strategies and testing innovative ones aimed at preventing and managing cardiovascular disease and diabetes in high-burden populations. RIDHDS has identified as its priority population adults who are below 400% of the Federal Poverty Level, who are at heightened risk for both development and poor control of chronic disease. 1817 will continue to focus on this priority population, with a heightened focus on reducing racial/ethnic disparities in the state. RIDHDS is organized into three main areas of work: 1) Health Systems Intervention (HSI); 2) Community Health Network (CHN), which serves as the community-clinical linkage mechanism; and 3) evidence-based lifestyle programs (EBLP). RIDHDS will address RI’s chronic disease burden in priority populations by leveraging the CHN, community health workers (CHW), and EBLP partners to continue to grow and improve the infrastructure of EBLPs, including integrating EBLPs into Health Care Organizations and targeting barriers to participation. RIDHDS will simultaneously address HSI by working with its ongoing and new partners to build on accomplishments under 1305/1422 in EHR utilization, population health management, and team-based care optimization through working synergistically with 1815 activities to expand the scale and scope of work done with and by partner Health Equity Zones and CBOs, with particular focus on expanding CHN capacities, expanding use of non-physician team members, care practices, and telehealth to improve clinical reach. For Category A, this approach will lead to the following outcomes by the end of the project period: 1) Increased access to the National DPP lifestyle change program in underserved areas for people with diabetes; 2) Increased community clinical links that support bi-directional referrals to assist in enrolling and retaining participants in the National DPP lifestyle change program; 3) Increased enrollment and retention in CDC-recognized organizations delivering the National DPP lifestyle change program; 4) Increased number of people with prediabetes enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss; 5) Increase adoption and use of clinical systems and care practices to improve health outcomes for people with diabetes; 6) Increased number of people with diabetes served by healthcare systems actively using clinical systems and care practices to improve diabetes-related health outcomes; and 7) Decreased proportion of people with diabetes with an A1C > 9. For Category B, this approach will lead to the following outcomes by the end of the project period: 1) Increased reporting, monitoring, and tracking of clinical data for improved identification, management, and treatment of patients with high blood pressure and high blood cholesterol; 2) Increased use of and adherence to evidence-based guidelines and policies related to team-based care for patients with high blood pressure and high blood cholesterol; 3) Increased community clinical links that support bi-directional referrals, self-management, and lifestyle change for patients with high blood pressure, high blood cholesterol, and/or who have had a cardiac event; 4) Increased medication adherence among patients with high blood pressure and high blood cholesterol; 5) Increased engagement in self-management among patients with high blood pressure and high blood cholesterol; 6) Increased participation in evidence-based lifestyle interventions among patients with high blood pressu re and high blood cholesterol; 7) Increased control among adults with known high blood pressure; and 8) Increased cholesterol management among patients at high risk of cardiovascular events.
