The South Carolina Department of Health and Environmental Control, Division of Diabetes, Heart Disease and Stroke (DDHDS), requests funding to implement Category A and Category B for CDC-RFA-DP18-1817: Diabetes and Heart Disease & Stroke Prevent Programs-Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes and Heart Disease and Stroke to design, test and evaluate novel approaches to address the identified evidence-based strategies aimed at reducing the risks, complications and barriers to prevention and control of diabetes and CVD among high burden populations in the state. Funding will enhance and elevate statewide transformational changes implemented by South Carolina under DP18-1815, thereby optimizing a culture of sustainability. This application supports Category A: diabetes management and type 2 diabetes prevention strategies and Category B: CVD prevention and management strategies statewide, and in local counties disproportionately affected these chronic conditions, and where significant reach can be achieved. Building upon the accomplishments and outcomes achieved under CDC-RFA-DP13-1305 and CDC-RFA-DP14-1422, selected interventions support: patient engagement in primary and secondary prevention and control programs and increased healthy behaviors; and health systems improvements to maximize identification and treatment of prediabetes, diabetes and uncontrolled hypertension, and high cholesterol. The selected strategies and developed activities coordinate and integrate long-standing efforts, resources and partnerships by increasing the availability of NDPP and DSME programs statewide; increasing health care systems ability to identify and prevent cardiovascular disease, and build stronger community-clinical linkages; Where possible, SC programs will coordinate cost sharing of positions, workloads, planning, implementation and integration of objectives and activities, and data usage to support and maximi ze efficiencies of planned activities. Staff alignment with internal and external partners demonstrates our capacity to implement the proposal and successfully carry out planned strategies, objectives and activities.
