The Colorado Department of Public Health and Environment (CDPHE) will work in partnership with health systems, community-based organizations (CBOs) and academic institutions to design, implement and evaluate a coordinated set of mutually reinforcing innovative strategies aimed at reducing risks, complications and barriers for the prevention and management of diabetes and cardiovascular disease. These two diseases account for more deaths in Colorado each year than any other single cause and the problems from uncontrolled diabetes and hypertension appear to be increasing sharply in recent years. Colorado has numerous opportunities to improve the prevention and management of high blood pressure, high cholesterol and diabetes, particularly through health system advances and targeted approaches to reach disparately-affected populations. CDPHE will conduct work in nine, high burden, priority counties, with eight in the urban corridor of Colorado’s front range, and one rural county to test implementation in a different setting. Approximately 68 percent of the state’s adult (aged 18+) population, or 2,696,980 Coloradans, live in the priority counties and will be reached through this proposal. CDPHE will leverage infrastructure and past successes to engage partners across a total of 12 strategies, six from Category A and six from Category B, to substantially improve health outcomes and narrow health disparities for those with cardiovascular disease and diabetes through coordinated solutions. By conclusion of the five-year performance period, CDPHE will achieve the following four long-term outcomes: 1) an increased number of people with prediabetes enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss, 2) a decreased proportion of people with diabetes with an A1C > 9, 3) an increased control among adults with known high blood pressure, and 4) increased cholesterol management among patients at high risk of cardiovascular events. These outcomes will be achieved by applying technology-based tools and clinic quality improvement practices that facilitate referrals to chronic disease programming, expand support from non-traditional caregivers, increase access to disease management and prevention services, and support patient adherence. CDPHE will augment these efforts by promoting learning opportunities for health system staff that reinforce the purpose, value and application of community programs, team-based care and practice transformation strategies. CDPHE has extensive experience in health systems change and community-clinical linkages, and is committed to leveraging and advancing this experience to impact health systems caring for, and populations with, prediabetes, diabetes, high blood pressure, high cholesterol and/or a prior cardiac event. CDPHE will weave together multiple evidence-based strategies and interventions that will transform the relationship between clinics and communities and support the prevention and management of diabetes and cardiovascular disease in high burden communities in Colorado.
