According to the Michigan Behavioral Risk Factor Survey (MiBRFS), over 67.2% of adults in Michigan have one or more of the following conditions: diabetes, high blood pressure, heart disease, or obesity. An estimated 2.6 million people are affected by high blood pressure in Michigan, and 3.1 million are affected by high cholesterol. Health disparities exist and several racial/ethnic groups have disproportionately higher chronic disease burden than the non-Hispanic White population in Michigan. Arab/Chaldean, non-Hispanic Black, non-Hispanic American Indian, and Hispanic/Latino populations showed statistically significantly higher estimated prevalence of and mortality due to various chronic conditions when compared to the non-Hispanic White population. An estimated 250,000 adults reside in Michigan’s Upper Peninsula (UP) and approximately two-thirds (66.8%) have a chronic disease. This new funding opportunity will address both Category A and B strategies. The MDHHS Diabetes Prevention and Control Program (DPCP) has selected to work on six Category A strategies (A1-A5 and A8). The MDHHS Heart Disease and Stroke Prevention (HDSP) Unit will implement seven Category B strategies (B1-B3 and B5-B8). DPCP and HDSP will work with the same and unique partners to enable mutually reinforcing strategies affecting both diabetes and heart disease and stroke prevention. A focus of DPCP is on innovative strategies to recruit, enroll and retain people in the CDC-recognized lifestyle change programs and the ADA-recognized/AADE-accredited diabetes self-management education and support programs. HDSP key partnership with Wayne State University in Detroit, MI will facilitate an all-systems approach to addressing high blood pressure and cholesterol in health systems, spanning most of their Category B strategies. HDSP will also work on health information technology, electronic health records, and clinical and community preventive services. MDHHS has strong collaborations with internal and external partners and organizational capacity to implement and evaluate proposed strategies. DPCP and HDSP staff have well-established history of collective work with other MDHHS Departments, Divisions, Sections and Units. The tremendous experience in developing and maintaining strong networks and long-term partnerships are demonstrated by the breadth of their partnerships. Organizations represented include some of the state’s largest health systems, multiple medical professional organizations, universities, healthcare quality improvement organizations, state Medicaid, nonprofits offering DPP and DSMES lifestyle change programs, and Michigan’s health information exchange systems. The DPCP and HDSP Units will continue their effective and integrative approach through the implementation of mutually reinforcing strategies, common partnerships, technical assistance, communication initiatives, epidemiology and surveillance, evaluation, and fiscal management. Performance monitoring and rigorous evaluation outcomes at the end of this funding cycle will demonstrate: increased access to, enrollment in, and retention in evidence-based programs for people at risk of diabetes and/or living with diabetes or high blood pressure; increased engagement of pharmacists and Community Health Workers as members of clinical care teams; increased and improved provider usage of health information technology to improve patient outcomes and reduce health disparities; and improved health outcomes in weight loss, diabetes, heart disease, high blood pressure, and high cholesterol.
