Diabetes has a significant impact on physical health, quality of life, employment, and medical expenses in Massachusetts, resulting in $8 billion of direct and indirect healthcare and lost productivity costs in 2012 alone. Disparities in diabetes burden exist by race/ethnicity; while Black adults and Hispanic adults were slightly more at risk for ever-diagnosis with diabetes than were White non-Hispanic adults, rates of hospitalization for diabetes among each of these populations was significantly higher than those for White non-Hispanic adults after adjusting for age. Heart disease and stroke also have substantial impact in Massachusetts, with significant inequities along racial lines: Black, non-Hispanic adults and Hispanic adults experience higher rates of hospitalization for cardiovascular disease and stroke than White non-Hispanic adults. When determining where chronic disease prevention and management efforts should be focused, it is critical to consider inequities that exist at the community level. Massachusetts has significant inequities among its 351 communities as shown by indicators such as socioeconomic status, environment, and race/ethnicity; risk factors associated with poor chronic disease health outcomes such as exercise and insurance type; and health outcomes such as obesity, diabetes, and heart conditions. Through a partnership among the Massachusetts Department of Public Health (MDPH) and five community health centers subawardees selected for their demonstrated reach, experience, and expertise serving populations most at risk for diabetes and cardiovascular disease, this work will use a racial justice lens to build upon significant past successes and innovation in Massachusetts linking clinical and community settings and conducting coordinated work in the areas of diabetes management, type 2 diabetes prevention, and cardiovascular disease prevention and management. By leveraging unique state data systems connecting clinical and community settings, electronic health record (EHR) data to identify populations of focus and engage in continuous quality improvement of clinical care, and working with community health workers and other non-physician team members to explore and overcome population- and community-specific barriers to care, we will use a collaborative, data-driven approach on both the state and community level to work toward eliminating the inequities experienced by communities of color who are disproportionately affected by these chronic diseases and their risk factors. The long-term outcomes for grant activities (with outcomes for priority populations in parenthesis) are to decrease the proportion of people with diabetes with an A1C >9 by 3% (among persons of color 3%), increase the number of people with prediabetes enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss by 2% (among persons of color 2%), increase blood pressure control among adults with known high blood pressure by 3% (among African Americans by 3%), increase control of high blood cholesterol by 2% (among African Americans by 3%), increase proportion of people with diabetes who have diagnosed retinopathy by 1%, and increase proportion of patients considered at high risk of cardiovascular events who have their cholesterol managed with statin therapy by 2% (among African Americans by 2%). In conclusion, this funding will build on MDPH’s substantial expertise and experience in chronic disease prevention and management to use innovative models to address root causes and improve management of diabetes and cardiovascular disease in the highest-need communities the state.
