Kentucky 1817 Grant - Executive Summary The Kentucky Diabetes Prevention and Control Program, which is part of the Kentucky Department for Public Health, has developed this proposal in collaboration with a group of partners considered key to success in designing, implementing and evaluating the work detailed in the work-plan. For Strategy A.1, we will collaborate with the National Association of Chronic Disease Directors, two large primary care practices, and two NDPP providers in Lexington, Kentucky to test the “Community e-connect” tool which allows bi-directional linkage between clinical electronic health records (EHRs) and community-based organizations (CBOs). Under strategy A.3, we will develop culturally responsive NDPP messaging tailored to residents and health care providers in our underserved, high burden Appalachian region. For strategy A.4 we focus on providing advanced training for NDPP lifestyle coaches across the state. For strategy A.5, assess we will identify community providers to test innovative ways to eliminate barriers to NDPP or DSMES participation, work with a multi-county district health department to implement telehealth delivery of DSMES. Finally, under strategy A.7 we establish a yearlong learning collaborative to improve use of Electronic Health Record (EHR) based clinical decision support tools and bi-directional referrals. This project targets two geographic areas of the state, Economically Distressed Appalachian counties (as defined by the Appalachian Regional Commission), and the adjoining Bluegrass region. Together, these regions have a population of just over 2,000,000. Some strategies and activities will be implemented in both geographic areas while other strategies will be focused more tightly on Appalachia. The proposed activities are designed to achieve the following outcomes as delineated in the NOFO: Diabetes Prevention: • Increased access to the National DPP lifestyle change program in underserved areas for people with prediabetes. • Increased community clinical linkages that support bi-directional referrals to assist in enrolling and retaining participants in the National DPP lifestyle change program. • Increased enrollment and retention in CDC-recognized organizations delivering the DPP lifestyle change program. • Increased number of people with prediabetes enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss. Diabetes Management: • Increased adoption and use of clinical systems and care practices to improve health outcomes for people with diabetes. • Increased number of people with diabetes served by healthcare systems actively using clinical systems and care practices to improved diabetes-related health outcomes. • Decreased proportion of people with diabetes with an A1C > 9.
