The burden of chronic disease remains high with cardiovascular disease and diabetes together accounting for 25 percent of all deaths in Minnesota and approximately $5 billion in combined inpatient hospitalizations and associated health care costs. Over the past 25 years, prevalence of diabetes and obesity in adults has more than doubled. A large proportion of our population remains unaware of their hypertension, prediabetes and diabetes. This lack of awareness occurs in combination with problems of access to clinical care and social and economic factors. We have unacceptable disparities in health outcomes across our populations. African American and American Indians are dying from chronic diseases at rates two to three times that of the general population. We attribute these disparities in health outcomes to differences in opportunities for achieving a solid education, earning a decent income, social isolation, and inadequate access to preventative health care. Health disparities in Minnesota are especially acute in lower income populations that include Medicaid enrollees, populations of color, and those with a serious mental illness, including severe depression. The greatest concentration of diversity and at-risk populations in Minnesota is within Hennepin and Ramsey Counties, including the cities of Minneapolis and Saint Paul, and will be the geographic catchment area of focus for this application. Purpose: MDH will design, test, and evaluate innovative approaches to address a set of evidence-based strategies aimed at reducing risks, complications, and barriers to prevention and control of diabetes and CVD in high-burden populations. The majority of this work will occur within the two adjacent, most heavily populated counties in Minnesota: Hennepin (pop. 1,184,576, 2012 Census figures) and Ramsey (pop. 520,152), in which Minneapolis (pop. 392,871) and Saint Paul (pop. 290,776) respectively reside. Short-term outcomes: (Diabetes) Increase access to the National DPP lifestyle change in underserved areas for people with prediabetes; Increase adoption and use of clinical systems of care practices to improve health outcomes for people with diabetes; (CVD) Increase reporting, monitoring, and tracking of clinical data for improved identification, management, and treatment of patients with high blood pressure and high blood cholesterol; Increase use of and adherence to evidence-based guidelines and policies related to team-based care for patients with high blood pressure and high blood cholesterol; Increase community clinical links that support bi-directional referrals, self-management, and lifestyle change for patients with high blood pressure, high blood cholesterol, and/or who have had a cardiac event. Intermediate outcomes: (Diabetes) Increase enrollment and retention in CDC-recognized organizations delivering National DPP lifestyle change program; Increase number of people with diabetes served by healthcare systems actively using clinical systems and care practices to improve diabetes-related health outcomes; (CVD) Increase medication adherence among patients with high blood pressure and high blood cholesterol; Increase participation in evidence-based lifestyle interventions among patients with high blood pressure and high blood cholesterol. Long-term outcomes: (Diabetes) Increase number of people with prediabetes enrolled in CDC-recognized lifestyle change program who have achieved 5-7% weight loss; Decrease proportion of people with diabetes with an A1C > 9; (CVD) Increase control among adults with known high blood pressure; Increase cholesterol management among patients at high risk of CV events.
