Montana (MT) Department of Public Health & Human Services (DPHHS) is applying for the CDC-RFA-DP18-1817 using innovative strategies to prevent and manage diabetes and heart disease and stroke in MT. These activities will be prioritized to the highest burden areas or specific sub-populations at high-risk that may otherwise be missed by these activities. The Montana Diabetes Program will address the burden of diabetes in MT by implementing Category A strategies through a comprehensive, coordinated, and innovative approach. This approach includes focusing on diabetes prevention and management through Diabetes Prevention Program sites (DPP), ADA-recognized/AADE-accredited Diabetes Self-Management Education and Support (DSMES) sites, and clinic partnerships and establishing community-clinical linkages through a web-based bi-directional referral system and mobile health-care services. Also, Category A strategies will address health disparities on MT American Indian (AI) Reservations and Federally Qualified Health Centers (FQHCs) and test innovative strategies to prevent or manage diabetes. For the chosen Category A strategies (A1-A8), MT DPHHS will: 1) explore and test innovative ways to increase enrollment in DPP and DSMES, 2) improve health outcomes of people with diabetes, and 3) increase referrals to community and clinical services and screenings for people with prediabetes and diabetes. Montana’s Category B activities will be aligned with the best practice strategies identified by CDC in “Best Practices for Cardiovascular Disease: A Guide to Effective Health Care System Interventions and Community Programs Linked to Clinical Services.” This includes focusing on blood pressure (BP) and cholesterol control with primary care clinics and/or community pharmacies that will work on quality improvement, team-based care, medication adherence, and BP/cholesterol self-management. For the chosen Category B strategies (B2-B8), MT DPHHS will: 1) build on existing partnerships with organizations such as cardiac rehabilitation facilities, Indian Health Service, primary care clinics, and Skaggs School of Pharmacy to improve BP and cholesterol management and control; and 2) implement new approaches for community-clinical linkages (e.g., CONNECT bi-directional referral system, ArcGeographic Information Sytems (GIS) Hub, and community health emergency medical services). In cooperation with CDC, MT DPHHS will rigorously evaluate two of the Category A strategies and two of the Category B strategies to inform the evidence base. This funding will allow MT DPHHS to further reduce the burden of diabetes, heart disease and stroke in the state through innovative strategies and diverse partnerships.
