Over the 1817 grant period, EPICC will address diabetes, heart disease and stroke, and related risk factors across categories and with internal and external statewide partners and the Centers for Disease Control and Prevention to improve outcomes. EPICC will work to increase the number of people with prediabetes enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss, decrease proportion of people with diabetes with an A1C > 9%, increase control among adults with known high BP and high blood cholesterol and increase cholesterol management among patients at high risk of cardiovascular events. EPICC will use evidence based interventions to develop activities within the specific CDC identified strategies below. Evaluation will be considered from the inception of the activities, used to direct efforts and make mid-course changes. Results will be shared with stakeholders and used for future planning. Existing partnerships will be maximized and new partnership developed. Work will be complimentary to 1815, 1807 and other CDC funding initiatives in Utah. In Category A EPICC will address the following strategies: Strategy 1 implements systems to facilitate bi-directional e-referral between healthcare systems and CDC-recognized lifestyle change programs for type 2 diabetes prevention. Strategy 2 supports organizations in increasing enrollment in existing CDC-recognized lifestyle change programs or establishing and sustaining new CDC recognized lifestyle change programs in underserved areas. Strategy 3 implements tailored communication/messaging to reach underserved populations at greatest risk for type 2 diabetes to increase awareness of prediabetes and the National DPP. Strategy 4 supports advanced training for lifestyle coaches working at CDC-recognized lifestyle change programs to strengthen skills needed to engage and retain participants. Strategy 5 explores and tests innovative ways to eliminate barriers to participation and retention in CDC-recognized lifestyle change programs for type 2 diabetes prevention and/or ADA-recognized/AADE-accredited diabetes self-management education and support (DSMES*) programs for diabetes management among high burden populations. Strategy 6 works with healthcare systems to establish or expand use of telehealth technology to increase access to ADA-recognized/AADE-accredited DSMES* programs for diabetes management. Strategy 7 increases adoption and use of clinical systems and care practices to improve health outcomes for people with diabetes (e.g., HIT/EHRs, clinical decision support tools, learning collaboratives to improve quality of care). In Category B EPICC will address the following strategies: Strategy 1 increases the identification of patients with undiagnosed hypertension using EHRs/HIT. Strategy 2 explores and tests innovative ways to promote the adoption of evidence based quality measurement at the provider level. Strategy 3 explores and tests innovative ways to engage non-physician team members in hypertension and cholesterol management in clinical settings. Strategy 4 promotes the adoption of MTM between community pharmacists and physicians for the purpose of managing high blood pressure, high blood cholesterol, and lifestyle modification. Strategy 5 facilitates engagement of patient navigators/community health workers in hypertension and cholesterol management in clinical and community settings. Strategy 7 explores and test innovative ways to expand use of telehealth including mobile health technology (e.g., smart apps, text messages) to promote management of hypertension and high blood cholesterol.
