Purpose: The New York State (NYS) Department of Health (DOH) proposes to continue momentum from previously achieved successes and advances in transforming health care delivery and promoting preventive care in NYS by implementing and rigorously evaluating innovative activities to prevent and control diabetes, heart disease and stroke in high burden communities in NYS. DOH will lead a coordinated approach with key collaborators to improve access to and participation in the National Diabetes Prevention Program (DPP), improve the care and management of people with diabetes, track and monitor clinical measures to identify and improve the quality of health care delivered to patients with high blood pressure (HBP) and high blood cholesterol (HBC), promote team-based care for patients with HBP and HBC and support bi-directional referrals, self-management and lifestyle change for patients with HBP, HBC, or who have had a recent cardiac event. To ensure population level reach and promote sustainability, DOH is proposing to use Qualified Entities (QEs) contributing to the State Health Information Network of New York (SHIN-NY) as critical partners in implementing selected strategies and engaging partners. Outcomes: DOH will make measurable improvements in the following Category A outcomes: For Type 2 Diabetes prevention, increased access to the DPP in underserved areas for people with prediabetes, increased community-clinical links (CCLs) supported by bi-directional referrals to assist in enrolling and retaining participants in DPP, increased enrollment and retention in DPP, increased number of people with prediabetes enrolled in DPP who have achieved 5-7% weight loss; for Diabetes Management, increase adoption and use of clinical systems and care practices to improve health outcomes for people with diabetes, increased number of people with diabetes served by healthcare systems actively using clinical systems and care practices to improve diabetes-related h ealth outcomes, decreased proportion of people with diabetes with an A1C > 9. For Category B, outcomes include increased reporting, monitoring, and tracking of clinical data for improved identification, management, and treatment of patients with HBP and HBC, increased use of and adherence to evidence-based guidelines and policies on team-based care for patients with HBP and HBC, increased CCLs supported by bi-directional referrals, self-management, and lifestyle change for patients with HBP, HBC, and/or who have had a cardiac event, increased medication adherence among patients with HBP and HBC, increased engagement in self-management among patients with HBP and HBC, increased participation in evidence-based lifestyle interventions among patients with HBP and HBC, increased control among adults with known HBP and increased cholesterol management among patients at high risk of cardiovascular events. Approach: To achieve these outcomes, DOH will 1) contribute to ongoing DOH initiatives transforming health care delivery, promoting preventive care, and addressing social determinants of health; 2) build upon successes achieved and lessons learned from previous diabetes and cardiovascular disease CDC-funded cooperative agreements; 3) ensure population level reach and sustainability by using QEs that contribute to the SHIN-NY as critical partners in implementing selected Category A and B strategies; 4) test and evaluate seven strategies in Year 1 with three QEs and scale implementation to all 8 QEs by Year 5; 5) follow Category A and B logic models and 6) maintain and expand NYS’ highly sophisticated chronic disease surveillance and performance measurement system for targeting and monitoring, and rigorously evaluate A.3, A.5, B.6 & B.8 strategies.
