PROPOSED POPULATION: Bernalillo County Healthy Here: Communities Leading Healthy Change will focus work on Priority Populations of Hispanic Americans and American Indians. These populations face higher local rates of heart disease, obesity, and Type 2 diabetes. We will also direct efforts at improving resource accessibility for people with disabilities and non-English speaking populations. Healthy Here is a current REACH-supported coalition and will use continued support to expand the previous two geographies and add a third geography. The selected areas are low-income parts in Bernalillo County where Priority Populations account for a combined 78% of residents. These are the South Valley, the International District, and the 2nd/4th Street Corridor. In comparison to the current REACH agreement, this area has a total population 30% higher than previous work (from 69,733 to 90,646).PURPOSE: Healthy Here will reduce health disparities among Hispanic and Native American populations living in three high-poverty urban and peri-urban areas in Bernalillo County, New Mexico. The applicant and coalition will use community-based participatory approaches to assess population needs, barriers, resources, and health characteristics, and to plan culturally-tailored and replicable interventions. The planned comprehensive approach will increase access points for fresh produce and reduce economic, logistic, and policy barriers to consumption; improve awareness and opportunities for physical activities?especially walking between and at anchor sites and workplaces; and spread the use of clinical-community connections through tools like a Wellness Referral Center.CORE ACTIVITIES: The project will reduce health disparities using the following strategies to improve health outcomes and healthy habits: 1) Improve access to and affordability of fresh produce through Mobile Farmer?s Market, food pharmacies, a local food hub, commercial and institutional policies and procur ement, and vouchers and benefits enrollment; 2) Establish and improve physical activity opportunities by creating new walking paths in public and workplace land, connecting community sites and clinics with safe walking routes, improving connectivity and affordability of bike share programs and bus transit, integrating pedestrian planning into regional master planning, and expanding use of employee health incentive programs; and3) Expand the use of the Wellness Referral Center, which allows providers to refer patients to community and public health supports like classes, activity programs, healthy food access, and breastfeeding resources. OUTCOMES AND IMPACT: Outcomes are: 1) Increased number of places offering healthier foods; (2) Increased number of persons with access to healthier foods; (3) Increased number of continuity of care/community support actions implemented for breastfeeding; (4) Increased number of places that improve community design by connecting safe and accessible places for physical activity; (5) Increased number of persons with safe and accessible places for physical activity; and (6) Increased use of appropriate and locally available health or community programs. In accomplishing these outcomes, Healthy Here will reach or benefit 52,800 people (75% of the priority populations living in the focus geographies).
