Purpose: The REACH Pima Partnership (RPP) brings together entities with a history of successful collaboration on health and wellness issues among American Indian (AI) and Mexican-American (MA) populations throughout Pima County. The partners will build on existing Community Action Coalitions to develop and implement culturally and socioeconomically tailored interventions that specifically engage Mexican-American and American Indian communities in Pima County, Arizona. Methodology, activities and programming will be designed to take into account the needs and socio-economic living conditions of the target population and will address tobacco use, poor nutrition and physical inactivity with a specific focus on school-aged children and their families. We aim to improve health, prevent chronic diseases, and reduce health disparities among our population at the highest risk/burden of chronic diseases including hypertension, heart disease, Type 2 diabetes, and obesity. Outcomes: We propose a series of activities to improve health outcomes and reduce health disparities in chronic conditions among Mexican-Americans and American Indians in Pima County Arizona through increases in tobacco-free living, sales of healthier foods, and increased physical activity through culturally and socioeconomically tailored, multi media campaigns, education and training, policy development, participation incentives and increasing access to existing programs and services. Activities/Methods: In an effort to decrease health disparities and promote health equity among American Indian and Mexican-Americans in Pima County, we will emphasize school-based initiatives, as both a place to reach youth and families but also as places of work to reach employees. Our methods include incentivizing positive health behaviors, building community capacity to develop and implement policies, and developing and implementing culturally competent messages and campaigns to motivate grassroots i nvolvement in policy development. Specific activities will include:1.Promoting and leveraging CDC?s national tobacco education campaigns targeting Mexican-American and American Indian populations with specific tobacco control messages and disseminate through multi-media outlets.2.Training tobacco retailers located near schools on preventing sales to minors.3.Training community health representatives (CHR?s), school personnel, healthcare and behavioral health providers on how to make tobacco cessation referrals.4.Build on policy efforts to implement and expand tobacco-free parks and recreational spaces. 5.Providing workshops on tobacco-free workplace and housing policy development. 6.Training CHRs on participant navigation to increase referrals to tobacco cessation, nutrition services and chronic disease management/prevention resources. 7.Develop and implement Mexican-American and American Indian specific nutrition information messages and disseminating through multi-media outlets.8.Develop and implement a model traditional foods menu and policy for school cafeterias.9.Facilitate the process for Farmer?s Market vendors to obtain State authorization to accept food vouchers10.Promote and recruit restaurants to participate in incentive program if they incorporate healthy options into their menu?s (salt, trans-fat, etc.)11.Promote the 130 mile LOOP (an interconnected system of paved pathways throughout Pima County) to support physical activity, recreation, and cultural learning for children and families served by adjacent schools and communities.12.Establishing a school-based program targeting mothers in poverty that focuses on self-efficacy, increased knowledge of health care systems and advocacy skills.13.Creating Pocket Parks near schools encouraging parents to walk kids to school.
