The Montgomery Area Community Wellness Coalition (dba The Wellness Coalition), in collaboration with community partners, will implement the REACH-River Region project, the purpose of which is to address disparities in the impact of chronic diseases in African Americans residing in 42 census tracts located in three counties of Central Alabama?s River Region (Lowndes, Macon, and Montgomery). Our selection of geographic area is based on the high percentage of African Americans in the selected census tracts, accompanied by high rates of poverty, low educational levels, and the high burden of chronic disease in the selected counties. The Coalition will use locally tailored evidence-based interventions in the areas of Tobacco, Nutrition, and Community-Clinical Linkages. Our selection of strategies is based on recent community health needs assessments and experience with our current REACH project, which will allow us to implement the interventions immediately upon award. We will design interventions to achieve outcomes to: (1) increase tobacco-free living through increasing referrals to the Alabama Department of Public Health Tobacco Quitline and by conducting an educational campaign based on the CDC Tobacco Toolkit, aimed at residents of public housing and local worksites; (2) improve nutrition through development of a Fresh Truck to travel to neighborhoods in food deserts, increasing workplaces and churches that support onsite breastfeeding, and delivering public and partner messages encouraging healthy eating; and (3) increase community-clinical linkages to the existing health resources and programs through placement of Wellness Navigators in clinics and county health departments, training existing staff in community agencies to provide Community Health Worker services and supporting those trained through a Community Health Worker network, increasing the number of trained Lay Leaders for Chronic Disease Self-Management Program, and engaging local p harmacists to make referrals to the Chronic Disease Self-Management Program, Diabetes Self-Management Program and Diabetes Prevention Program. The Wellness Coalition (TWC) is a non-profit consortium of ?safety net? health care and social service organizations offering planning and coordination of health care services for uninsured and underserved persons. We will work with community partners to develop and implement the REACH interventions, including The Wellness Coalition board member agencies (hospitals, health centers, mental health, health departments, homeless coalition and others); the River Region Obesity Task Force; Alabama Department of Public Health Chronic Disease Division, Tobacco Prevention and Control Branch, and Division of Nutrition and Physical Activity; the nonprofit Farmscape Solutions; and local African American churches and nonprofit community organizations.
