Local Efforts towards Addressing Disparities in DeKalb (LEAD DeKalb) COVID-19 Vaccination - Background and Problem Statement: DeKalb County, GA, the fourth largest county in Georgia, is ranked 18th overall for its health outcomes. Approximately 30% of adults and 13% of adolescents in the county are obese. Diabetes is the eighth leading cause of mortality in the county and affects 10% of adults. Furthermore, cardiovascular disease is the second leading cause of death in DeKalb County, GA. To reduce this disease burden and improve quality of life for DeKalb County residents, strategies that increase fruits and vegetable consumption, increase the frequency and duration of physical activity, and improve chronic disease management are needed. Approach and Outcomes: DeKalb County Board of Health (DCBOH) proposes to implement a 5-year initiative designed to improve nutrition, physical activity and community-clinical linkages within DeKalb County Ga. The Local Efforts towards Addressing Disparities in DeKalb (LEAD DeKalb) initiative will target African Americans communities to increase the number of persons with access to healthier foods, increase the number of places offering healthier foods, increase the number of continuity of community support actions implemented for breastfeeding, increase the number of persons with safe and accessible places for physical activity, increase the number of places that improve community design, and increase the use of appropriate and locally available health programs. Strategies and Activities: DCBOH will focus on nutrition, physical activity, and community-clinical linkages. As part of these 3 strategies, the proposed LEAD DeKalb initiative will implement 8 activities such as establishing a network of food sales outlets, making improvements to voucher incentive programs, improving nutrition standards, improving access to healthier foods at community venues, establishing lactation support services, establishing new or improved land use design, establishing new or improved pedestrian and bicycle transportation systems, promoting the use of appropriate and locally available health programs, and expanding the use of health professionals to increase referrals. Collaborations: To implement the proposed strategies and activities, DCBOH will engage eight partners from multiple sectors including the Live Healthy DeKalb Coalition. Target Population and Geographical Location: The target population for the proposed strategies will be African Americans (black, non-Hispanic), living in DeKalb County, Ga. DCBOH will target 39 of the 43 census tracts that have a population of at least 25% African American and have a population at least 20% with an income below the 100% federal poverty threshold. The total number of African Americans who are also below the 100% federal poverty threshold and will benefit from this project is over 70,000 residents. This equates to 22% of the African-American population (N=325,970), 78.3% of those below the 100% federal poverty threshold (N=91,707), and 9.5% of the total population in DeKalb County, GA (N=753,253). Budget: To implement Year 1 strategies and activities, DCBOH requests $900,000. The total requested funding in direct costs is $806,452. The total requested funding in indirect cost is $93,548.
