The overall purpose of the project is to support Mississippi Gulf Coast communities to implement strategies that improve health, prevent chronic disease, and reduce health disparities among African Americans who have the highest burden of chronic disease related to health risk behaviors. The project will implement evidence-based interventions supported by a strong community-level stakeholder coalition. Interventions aimed at women of child-bearing age and their families will improve birth outcomes, maternal health, infant health and effectively influence the community to support improved nutrition with an emphasis on breastfeeding and support maternal and family tobacco free living. Expected outcomes include a reduction in health disparities among the target population, reduced chronic disease, and improved clinical and community linkages.The Mississippi Public Health Institute proposes to implement a project that will focus on the health and well-being of African American families, mothers and babies in Hancock, Harrison and Jackson counties. This project will engage partners through collaborative activities of a community coalition that will provide opportunity for genuine community-level stakeholder involvement and be a force for positive change in the health of their communities. While each partner will retain its identity, members will collectively utilize community specific best practices to address health disparities, monitor progress and oversee communications within their communities to address risk factors related to chronic disease.The project will: (1) support tobacco free living by increasing community knowledge about the health effects of smoking and exposure to second-hand smoke, facilitating linkages to tobacco cessation support and increasing the number of primary care providers who integrate tobacco cessation strategies into practice. (2) improve nutrient intake by increasing community support for breastfeeding through implemen tation of Baby Cafes in the target area through a cluster model, increase the number of primary care providers who link mothers to lactation support services and through a focused communication campaign that influences health behaviors.
