The American Heart Association/American Stroke Association (AHA/ASA) has a long-standing commitment to both translate science into public health action, policy and community-based supports as well as mobilize resources at the national, state and community levels to encourage decision-makers to adopt science-based policies and programs. The AHA/ASA?s seeks to improve the health of all Americans by supporting the adoption of and implementing policy, systems and environmental (PSE) changes at the federal, state and local levels to reduce health inequities and address health disparities for vulnerable populations nationwide. It is the intent of the AHA/ASA through the proposed Heart Racial and Ethnic Approaches to Community Health (Heart REACH) program to leverage our demonstrated expertise to implement prevention and population-based strategies that will prevent chronic diseases and reduce health disparities. We will work to build the capacity of local coalitions in the County of El Paso, Texas to implement evidence-based activities, as well as public health and population-based strategies that create PSE improvements across three areas of focus: tobacco; nutrition and physical activity. We will also evaluate program outcomes and improved health outcomes.The AHA/ASA will collaborate with the CDC, local coalition partners and other CDC REACH awardees to deliver activities and interventions that support the CDC?s goals, objectives and desired outcomes. This funding opportunity aligns with AHA/ASA?s strategic priorities and we will seek to replicate our past successes in saving even more lives. We will accomplish this through local collaborations and partnerships in the county of El Paso, specifically with the HEAL Alliance and A Smoke Free El Paso Del Norte, and by targeting our work toward underserved Hispanic/Latino and African American communities where we can make the greatest impact.The AHA/ASA?s Health REACH program team will work with our Sou thwest Affiliate staff to enhance and strengthen the capacity of the existing coalitions and their respective strategic plans to accelerate local priorities. We will work collaboratively to mobilize our coalitions into intensified action through development of community action plans that will drive strategies to improve health outcomes. Overall, the Heart REACH program goal is to make a lasting impression on reducing health inequities, health disparities, suffering, disability and premature death of the residents in the county of El Paso.
