The Southern Nevada Health District (SNHD), located in Clark County, Nevada is proposing the Southern Nevada Health District Community Partnership to Promote Health Equity project. The Project follows the CDC REACH logic model framework and utilizes coalition partners actively engaged in program planning, implementation, and evaluation. The proposed Work Plan is intended to contribute to decreasing or eliminating health disparities among African American/Black and Hispanic residents (the priority populations) in Clark County, Nevada.Findings from the 2016 Southern Nevada Community Health Assessment were used to identify the priority populations and identify chronic disease rates, risk factors, and disparities. To reach the priority populations, we have identified zip codes where high concentrations of Non-Hispanic Blacks and Hispanics reside and where more than 20% of the population lives at or below the federal poverty level. While several of our initiatives will have community-wide impact, all strategies and activities will focus on residents living, working, or attending school in these geographic areas.Our Work Plan uses SMART objectives and evidence-based strategies that will be utilized to achieve project outcomes. Each strategy will utilize culturally and linguistically tailored approaches. The Work Plan addresses four CDC- identified chronic disease risk factors through 16 objectives implemented in community, school and worksite settings within priority zip codes. Proposed outcomes for Year 1 include: Tobacco: Increasing call volume among priority populations to the Nevada Tobacco Users Quitline (Quitline); Increasing delivery of brief tobacco-use interventions by healthcare providers; Increasing education at community events; Increasing worksites and higher education institutions with tobacco-free policies; Increasing availability of smoke-free multiunit housing; and development of healthy tobacco retail environments through retail asse ssments. Nutrition: Working towards adoption of nutrition standards for a food pantry; Completing a market study for a mixed-use co-op grocery store; Increasing the number of vending machines in compliance with nutrition standards, promoting increased access to healthier foods, and adoption of healthy food resolutions; and Increasing the number of Certified Lactation Consultants in programs serving priority populations; Physical Activity: Conducing a Health Impact Assessment to prioritize pedestrian improvement projects in priority zip codes; and Conducting an assessment of park access in priority zip codes to identify barriers and ensure walkability; Community Clinical Linkages: Securing electronic referral to the Quitline from a healthcare provider using an Electronic Health Record; and Increasing the number of Community Health Workers supporting referral to community programs. During Years 2-5, SNHD will expand on Year 1 successes and maintain the infrastructure components necessary to meet 5-year objectives.The project will be housed in the SNHD?s Office of Chronic Disease Prevention and Health Promotion (CDPHP). CDPHP will draw upon the resources of the project?s proposed community coalition, titled the Nevada Minority Health and Equity Coalition (NMHEC). NMHEC?s mission is to eliminate health disparities and to improve the health and well-being of underserved Nevadans. The NMHEC will work in partnership with a network of other local coalitions, community partners and SNHD to employ evidence and population-based strategies using a Community Based Participatory Approach.SNHD has assembled a well-qualified and highly motivated set of public health professionals, coalitions and contractors with demonstrated capacity in communications, community engagement, health equity, programming, administration, evaluation and fiscal management necessary to ensure successful implementation of the proposed Work Plan and, ultimately, improve healt
